Biofilm formation on dental implant surfaces is a serious threat. Up to 50% of all implants show signs of irreversible tissue destruction. The aim of the present systematic review was to summarize the state of the art of strategies to functionalize antimicrobial dental implant surfaces. We searched the following electronic database: SCOPUS, MEDLINE and GOOGLE SCHOLAR and identified relevant controlled trials that evaluated the efficiency of new biomaterial strategies to modify dental implant surfaces, in such a way that biofilm formation was inhibited. The search yielded 2,990 potentially relevant publications. A total of 142 publications met the inclusion criteria. Analysis found that it may be concluded that silver-implanted surfaces, drug-loaded surfaces, surfaces with antimicrobial peptides, bioactive and biopassive polymer coatings as well as nanoscale or UV-activatable surfaces enhance antimicrobial activity compared to commercial pure titanium.

To evaluate how guided and navigation surgical approaches for implant placement affect survival and accuracy. An electronic literature search was conducted in PubMed/Medline and the Cochrane Library. The reviews were refereed by two independent investigators using the following PICO question: population-patients with missing maxillary or mandibular teeth; intervention-dental implant guided surgery, dental implant navigation surgery; comparison-conventional implant surgery or historical control; outcome-implant survival, implant accuracy. Single-arm, weighted meta-analyses were performed on navigational and static guided surgery groups for cumulative survival rate and accuracy of implant placement (ie, angular, depth, and horizontal deviation). Group metrics with less than five reports were not synthesized. The study was compiled under PRISMA 2020 guidelines. A total of 3,930 articles were screened. Full-text review of 93 articles resulted in a total of 56 articles included for quantitative synthesis and analysis. Implant placement with a fully guided approach resulted in the following means and 95% CI: cumulative survival rate of 97% (96%, 98%), angular deviation of 3.8 degrees (3.4 degrees, 4.2 degrees), depth deviation of 0.5 mm (0.4 mm, 0.6 mm), and horizontal deviation at the implant neck of 1.2 mm (1.0 mm, 1.3 mm). Implant placement with a navigation approach resulted in an angular deviation of 3.4 degrees (3.0 degrees, 3.9 degrees), horizontal deviation at the implant neck of 0.9 mm (0.8 mm, 1.0 mm), and horizontal deviation at the implant apex of 1.2 mm (0.8 mm, 1.5 mm). Static guided and navigation surgical approaches for dental implant placement have survival rates comparable to historical controls. Accuracy of implant placement does not differ markedly between these two approaches.

AIM
To assess the impact of mucoperiosteal flap elevation for single immediate implant placement (IIP) on buccal hard and soft tissue changes, and on clinical, aesthetic and patient-reported outcomes.
MATERIALS AND METHODS
Two independent reviewers conducted an electronic literature search in Pubmed, Web of Science, Embase and Cochrane databases as well as a manual search to identify eligible clinical studies up to June 2022. Randomized controlled trials (RCTs) comparing IIP without flap elevation to IIP with flap elevation were included for a qualitative and quantitative analysis. The primary outcome was horizontal buccal bone change. Secondary outcomes were implant survival, vertical buccal bone change, pain, and clinical and aesthetic parameters.
RESULTS
Out of 1029 records, 5 RCTs were selected reporting on 140 patients who received 140 single immediate implants (flapless: 68; flap: 72). Patients had a mean age ranging from 30 to 67 years and were followed between 6 and 12 months. Four RCTs pertained to (nearly) intact alveoli. Risk of bias assessment yielded low risk for two RCTs and high risk for three RCTs. Meta-analysis demonstrated a mean difference of 0.48 mm (95% confidence interval [CI] [0.13, 0.84], p = .007) in horizontal buccal bone change between surgical approaches, favouring flapless surgery. Meta-analysis failed to demonstrate a significant difference in implant survival between the groups (RR 1.00, 95% CI [0.93, 1.07], p = .920). Given the scarcity of data, meta-analyses could not be performed on other secondary outcomes. Available studies were consistent in the direction of the effect favouring flapless surgery for vertical buccal bone change as well as for pain. Clinical and aesthetic parameters were underreported.
CONCLUSIONS
Based on CBCT data, flapless surgery resulted in more buccal bone preservation at immediate implants. However, the clinical relevance of this finding is unclear, since clinical and aesthetic outcomes were underreported.

STATEMENT OF PROBLEM
Immediate implant placement provides a popular therapeutic option. However, compromised sockets may jeopardize the treatment outcome.
PURPOSE
The purpose of this systematic review and meta-analysis was to investigate the treatment outcome in terms of the implant survival rate and success parameters of immediate implant placement in compromised extraction sockets.
MATERIAL AND METHODS
An electronic search was conducted in PubMed, Embase, Cochrane Library, and ISI Web of Science up to January 2021. Studies evaluating implant survival rate and main success parameters were included for a qualitative and quantitative analysis (risk ratio and mean difference).
RESULTS
In total, 43 studies with analysis of 4825 sockets were included. Compared with the noncompromised sockets, the compromised group showed no significant differences in implant survival rates (risk ratio=0.992; 95% confidence interval (CI)=0.979 to 1.005; P=.246). No significant statistical differences were found in marginal bone level at ≤12 months (mean difference [MD]=0.033; 95% CI=-0.012 to 0.078; P=.154) or esthetic parameters.
CONCLUSIONS
Immediate implant placement in compromised sites does not appear to decrease the survival and success rates. However, randomized clinical trials with large sample sizes should be conducted to draw a definite conclusion about the efficacy and safety of this treatment protocol in compromised sockets.

PURPOSE
The aim of this study was to test the null hypothesis that there is no difference in implant failure rates, marginal bone loss, and postoperative infection for implants inserted in bone with different qualities and quantities according to the classification of Lekholm and Zarb.
MATERIALS AND METHODS
An electronic search was undertaken in January 2015 for randomized and nonrandomized human clinical studies.
RESULTS
A total of 94 publications were included. When bone sites of different qualities were considered, the results suggested the following comparative implant failure rates: 1 > 2, 1 > 3, 3 > 2, 4 > 1, 4 > 2, and 4 > 3. Sensitivity analyses suggested that when implants inserted in bone qualities 1 and 2 and 1 and 3 were compared, oxidized and sandblasted/acid-etched surfaces showed a decrease in significant difference in failures compared with turned implants. The same is not true for failure of implants inserted in bone quality 4 compared to failure of implants in all other bone qualities. When bone sites of different quantities were considered, the following comparative implant failure rates were observed: A > B, A > C, A < D, B < C, B < D, C < D, E > A, E > B, E > C, E > D. Due to insufficient information, meta-analyses for the outcomes postoperative infection and marginal bone loss were not performed.
CONCLUSION
Sites with poorer bone quality and lack of bone volume may statistically affect implant failure rates. Implant surfaces may play a role in failure of implants in different bone qualities.

During the last decades, the number of immunosuppressed organ transplant patients has increased consistently. Nevertheless, immunosuppression has been discussed as a contraindication for dental implant procedures for many years. Hence, the purpose of this systematic review was to assess the survival rate and outcomes of dental implants after solid organ transplantation. An electronic and manual literature search was conducted up to March 2021. Publications describing dental implants placed in patients after organ transplantation were included without any limitations regarding study design or date of publication. Ten articles met the inclusion criteria, leading to a sample of 93 patients with 249 implants. Implant survival rates were 100% over a mean follow-up of 60 months. In every case, implant surgery was performed under antibiotic coverage. No major medication-related complications were reported. Despite the limited amount of evidence in the literature, implant procedures seem to be a safe treatment option in immunosuppressed organ transplant patients. The observance of appropriate treatment protocols including a strict maintenance programme seems to be crucial for the long-term success of such treatments. However, stringent data regarding various influencing factors such as the prevalence of peri-implantitis are still missing.

PURPOSE
This systematic review aimed to determine whether differences in the macro-geometry of titanium implants promote changes in osseointegration.
MATERIAL AND METHOD
SCOPUS, PubMed/Medline, Web of Science, and EMBASE databases were searched in June 2021. In addition, it was performed a manual search of the reference lists of the included articles. Eligibility criteria were in vivo studies that addressed the effect of titanium implant macro-geometry on osseointegration, studies that evaluated periodontally healthy models, and papers indexed in Journal Citation Reports.
RESULTS
The database search resulted in 1037 articles. Of the 19 articles selected for full reading, 16 remained in this systematic review. These had a high heterogeneity making it hard to perform statistical analysis of the data, so a descriptive analysis was performed.
CONCLUSIONS
Based on the studies included in this systematic review, implant macro-geometry provides influences on osseointegration. In this sense, the various isolated characteristics (thread type, thread pitch, thread depth, face angle) should be studied so that the implant geometry can balance the compressive stress and tensile stress and produce a minimum shear force.

The aim of this systematic review was to evaluate implant loss in younger and older patients. An electronic search of four databases (MEDLINE, EMBASE, SCOPUS and the Cochrane Library) was undertaken until May 2016 without time restriction and was supplemented by manual searching. Prospective cohorts were included if they met the following criteria: (i) presence of an exposed group (older subjects) with a minimum age of 60 years; (ii) presence of a control group (younger subjects) with a maximum age of 59 years; and (iii) outcome data considering implant survival or loss. Meta-analyses were performed to evaluate the impact of ageing on implant failure. Of 4152 potentially eligible articles, four were included in the qualitative analysis and quantitative synthesis. The pooled estimates suggest that the risk of implant loss in older patients is not significantly higher (RR = 0·92; 95% CI 0·43-1·96, P = 0·83) when compared to younger subjects. This systematic review suggests that age is not a limiting factor for dental implant therapy.

To answer the following PECO question: "In systemically healthy human subjects (P), which are the differences between peri-implantitis (E) and peri-implant health/mucositis (C) in terms of bacterial presence/count (O)?"
MATERIALS AND METHODS
Cross-sectional studies fulfilling specific inclusion criteria established to answer the PECO question were included. Two review authors independently searched for studies, screened the titles and abstracts, did full-text analysis, extracted the data from the included reports, and performed the risk of bias assessment through an adaptation of the Newcastle/Ottawa tool for cross-sectional studies and of the JBI critical appraisal checklist. In case of disagreement, a third reviewer author took the final decision. Study results were summarized using random effects meta-analyses.
RESULTS
A total of 12 studies were included, involving 1233 participants and 1513 implants. Peri-implantitis was associated with the presence of S. epidermidis (Odds ratio, OR = 10.28 [95% Confidence interval, CI: 1.26-83.98]), F. nucleatum (OR = 7.83 [95% CI: 2.24-27.36]), T. denticola (OR = 6.11 [95% CI: 2.72-13.76]), T. forsythia (OR = 4.25 [95% CI: 1.71-10.57]), P. intermedia (OR = 3.79 [95% CI: 1.07-13.35]), and P. gingivalis (OR = 2.46 [95% CI: 1.21-5.00]). Conversely, the presence of A. actinomycetemcomitans (OR = 3.82 [95% CI: 0.59-24.68]), S. aureus (OR = 1.05 [95% CI: 0.06-17.08]), and C. rectus (OR = 1.48 [95% CI: 0.69-3.17]) was not associated with peri-implantitis.
CONCLUSIONS
Peri-implantitis is associated with the presence of S. epidermidis and specific periodontopathogens (P. gingivalis, T. forsythia, T. denticola, F. nucleatum, and P. intermedia). (CRD42021254589).

OBJECTIVE
Titanium particles have been shown in in-vitro studies to lead to the activation of specific pathways, this work aims to systematically review in- vivo studies examining peri-implant and periodontal tissues at the transcriptome, proteome, epigenome and genome level to reveal implant material-related processes favoring peri-implantitis development investigated in animal and human trials.
METHODS
Inquiring three literature databases (Medline, Embase, Cochrane) a systematic search based on a priori defined PICOs was conducted: '-omics' studies comparing molecular signatures in healthy and infected peri-implant sites and/or healthy and periodontitis-affected teeth in animals/humans. After risk of bias assessments, lists of differentially expressed genes and results of functional enrichment analyses were compiled whenever possible.
RESULTS
Out of 2187 screened articles 9 publications were deemed eligible. Both healthy and inflamed peri-implant tissues showed distinct gene expression patterns compared to healthy/diseased periodontal tissues in animal (n = 4) or human studies (n = 5), with immune response, bone metabolism and oxidative stress being affected the most. Due to the lack of available re-analyzable data and inconsistency in methodology of the eligible studies, integrative analyses on differential gene expression were not applicable CONCLUSION: The differences of transcriptomic signatures in between peri-implant lesions compared to periodontal tissue might be related to titanium particles arising from dental implants and are in line with the in-vitro data recently published by our group. Nevertheless, limitations emerge from small sample sizes of included studies and insufficient publication of re-analyzable data.
