OBJECTIVE
Aim of this systematic review was to assess the clinical performance of sealants on various teeth in an evidence-based manner.
SOURCES
Five databases were searched from inception to February 2017.
DATA
Randomized clinical studies on humans.
METHODS
After duplicate study selection, data extraction, and risk of bias assessment according to the Cochrane guidelines, Paule-Mandel random-effects meta-analyses of Relative Risks (RRs) and their 95% confidence intervals (CIs) were calculated.
RESULTS
A total of 16 randomized clinical trials with 2778 patients (male/female 49.1%/50.9%) and an average age of 8.4 years were included. No significant difference in either caries incidence of sealed teeth or sealant retention could be found according to (i) mouth side (right versus left), (ii) jaw (upper versus lower), (iii) and tooth type (1st permanent molar versus 2nd permanent molar/1st permanent molar versus 2nd deciduous molar/1st deciduous molar versus 2nd deciduous molar), based on evidence of very low to low quality. On the other side, compared to 1st permanent molars, sealed premolars were significantly less likely to develop caries (3 trials; RR=0.12; 95% CI=0.03 to 0.44; P=0.001) and less likely to experience loss of the sealant (5 trials; RR=0.33; 95% CI=0.20 to 0.54; P=0.001), both based on low to moderate quality evidence.
CONCLUSIONS
The performance of pit and fissure sealants does not seem to be negatively affected by mouth side, jaw, and tooth type, apart from the exception of a favorable retention on premolars.
CLINICAL SIGNIFICANCE
Based on existing evidence, pit and fissure sealants can be effectively applied on any deciduous or permanent posterior teeth without adverse effects on their clinical performance.

Comparison and Evaluation of the Retention, Cariostatic Effect, and Discoloration of Conventional Clinpro 3M ESPE and Hydrophilic Ultraseal XT Hydro among 12-15-year-old Schoolchildren for a Period of 6 Months: A Single-blind Randomized Clinical Trial.
INTRODUCTION
Pit and fissures are more prone to caries as compared to smooth surfaces. Sealing the pit and fissures with sealants is considered to be highly effective in the prevention of pit and fissure caries. Hydrophobic sealants are technique sensitive in nature. Ultraseal XT Hydro sealant is moisture tolerant which incorporates the benefits of both hydrophilic and hydrophobic sealants into one unique chemistry. Hence, the study was conducted to compare and evaluate the retention, cariostatic effect, and discoloration of conventional Clinpro™ 3M™ ESPE™ and hydrophilic Ultraseal XT Hydro sealants among 12-15-year-old schoolchildren for 6 months.
MATERIALS AND METHODS
It was a single-blinded, randomized split-mouth clinical trial. Schoolchildren aged 12-15 years were recruited in this clinical trial. Clinpro sealant and Ultraseal XT Hydro were placed in the right and left quadrant molars, respectively, and were assigned as group I and II. The sample size arrived was 60 teeth per group and at 30 patients based on the computer-generated random sequence. Sealant coverage between the two sealants was compared using Mann-Whitney test and test for proportions.
RESULTS
After 6 months, it was seen that 10 teeth out of 44 (22.72%) with Clinpro had the sealant covering all the fissures compared to Ultraseal XT Hydro which was only 9%. Fifty-nine percent of retention rate was seen with Clinpro compared to Ultraseal XT Hydro sealant (27.27%). Caries incidence was higher in the case of group II. No difference was observed with regard to the discoloration of the sealants.
CONCLUSION
It was concluded that for a longer follow-up period conventional Clinpro™ 3M™ ESPE™ was better than Ultra XT Hydro seal in terms of retention and cariostatic effect.

BACKGROUND
Highly necessary to evaluate the effectiveness of preventive interventions to prioritize them at the community level. We aimed to systematically investigate the related studies on the effects of fluoride varnish and fissure sealant on dental caries in 6-12 children.
METHODS
We searched PubMed, EMBASE, Web of Science, Cochrane Library and Scopus databases using Fluoride Varnish, Fissure Sealant, Caries, and Oral Health keywords. The timeframe selected to search for articles is from 2000 to Dec 2020. CMA software: 2 (Comprehensive Meta-Analysis) was used to perform the meta-analysis. The intervention groups in this study were fluoride varnish and fissure sealants, each of them compared to the control groups.
RESULTS
We included nine studies. In the intervention group 84,380 and control group 11,254 individuals were studied. Eight of the studies were Randomized Clinical Trial (RCT) and Field RCT, and two was non-RCT. In the overall Fluoride Varnish efficacy study, 4 were fully effective, 1 was ineffective, and all 4 were completely effective for Fissure Sealant. There was a significant difference between decayed, missing, and filled permanent teeth (DMFT) and decayed, missing, and filled primary teeth (dmft) indices in both interventions and comparison groups. Moreover, the mean difference of DMFT for Fluoride Varnish and Fissure Sealant in the intervention and control groups were -0.55 and -0.29, respectively (=0.00).
CONCLUSION
Due to the efficacy of fissure sealant and fluoride varnish in preventing dental caries in children aged 6-12 yr, these interventions can be considered as health priorities of societies and health systems interventions in countries.

OBJECTIVES
This study aimed to evaluate the influence of pre-photoactivation temperature on the marginal and internal integrity (occurrence of voids) of fissure-sealing materials on occlusal fissures using optical coherence tomography (OCT).
METHODS
Occlusal fissures of 40 human third molars were sealed using a resin-based fissure sealant (Fluroshield) and a flowable composite (Permaflo) photoactivated at 68 °C (preheated) or at room temperature (25 °C) (n=10). After sealing, the teeth were subjected to thermocycling (500 cycles, 5-55 °C) and 14 days of pH cycling (demineralisation for 6 hours/day and remineralisation for 18 hours/day). The occlusal surfaces were scanned in a buccolingual direction, and 20 tomographic images parallel to the long axis of each tooth were obtained. Images presenting marginal gaps and internal voids were counted and statistically analysed using analysis of variance and Tukey's test (P<0.05).
RESULTS
There were statistically significant differences between materials and temperatures (P<0.01). The flowable composite had a lower percentage of gaps and internal voids than the resin-based sealant. Preheated materials had a lower percentage of gaps and internal voids than the materials at room temperature.
CONCLUSION
Therefore, preheated flowable composite provided the best marginal sealing of fissures, and internal homogeneity of the material.

BACKGROUND
This study aimed to evaluate the predisposition of microleakage in permanent molar teeth following different preparation techniques for pits and fissure sealants.
METHODS
In this cross-sectional analytical study, a dye penetration method was employed to evaluate microleakage in dental restorations. A total of 104 extracted molars were randomly assigned into two groups and further subdivided into two subgroups based on a class of sealant (filled/unfilled resin) containing 26 teeth each. Teeth in one group were prepared by a conventional method using pumice and acid etching with 37% phosphoric acid, and teeth in the other group were prepared with a 1/4-round carbide bur in a low-speed handpiece and then acid etched. In each group, 26 teeth were sealed with a filled sealant, and 26 teeth with an unfilled sealant. The chi-square test was used for the comparison of microleakage between the groups.
RESULTS
Unfilled sealants prepared with bur preparation showed the lowest degree of microleakage. The greater number of teeth (17) showed no microleakage in the group of teeth prepared using the bur preparation technique. Furthermore, in dye penetration analysis, the subgroup of teeth that were filled with unfilled sealants showed a greater number of teeth (57.6%), with zero penetration, and the dye penetration scores were statistically significant among the groups with different preparation techniques (p=0.002).
CONCLUSION
Teeth whose pits and fissures were prepared through 1/4 carbide bur and filled with unfilled pits and fissure sealants show less microleakage than those prepared with the conventional method and filled with filled sealants. Therefore, the choice of surface preparation technique for pit and fissure sealants can influence the effectiveness of the sealant in preventing microleakage.

BACKGROUND
The authors of this systematic review and meta-analysis had 2 aims: to evaluate fissure sealant retention with and without the use of an adhesive system and to compare fissure sealant retention using etch-and-rinse adhesive systems versus self-etching adhesive systems.
TYPES OF STUDIES REVIEWED
The authors conducted a literature search (all articles published through November 1, 2015) to identify studies for inclusion in this systematic review. They assessed the quality of the evidence provided using the modified Jadad scale and performed meta-analyses using a random-effects model.
RESULTS
The authors considered 12 studies that met the inclusion criteria for the systematic review. In addition, they used 5 of 9 studies related to the first part of the study and 3 of 4 studies related to the second part of the study that met the inclusion criteria for meta-analysis. In the analysis of the first part of the systematic review, the authors found that adhesive systems had a significant positive effect on fissure sealant (odds ratio, 3.294; 95% confidence interval, 1.292-8.401; P = .013). In the analysis of the second part of the systematic review, the authors found that etch-and-rinse adhesives were superior to self-etching adhesives in the fissure sealant procedure (odds ratio, 14.569; 95% confidence interval, 2.616-81.131; P = .002).
CONCLUSIONS AND PRACTICAL IMPLICATIONS
The use of adhesive systems beneath fissure sealants can increase the retention of fissure sealants. Also, when adhesive systems are used with fissure sealants, etch-and-rinse systems are preferable.

The pits and fissures of teeth have high caries susceptibility, and sealing these areas is considered as an effective method to prevent caries. However, long-term caries prophylaxis cannot be maintained because of the negative effects derived from the technical sensitivity and disadvantages of sealing materials. Herein, a new strategy is proposed to occlude fossae by amyloid-mediated biomimetic remineralization. In contrast to conventional inward blocking from the outside of fossae, amyloid-mediated biomimetic mineralization delivers an amyloid-like protein nanofilm into the deepest zone of the fossae and induces the formation of remineralized enamel inside. Such assembly from lysozyme conjugated with poly (ethylene glycol) enriches the interface with strongly bonded ionsand directs in situ nucleation to achieve enamel epitaxial growth. Not only is the structure of the enamel-like crystalline hydroxyapatite layer but also its mechanical stability is similar to that of natural enamel. Furthermore, the layer shows good biocompatibility and antibacterial properties. On the basis of the findings, it is demonstrated that amyloid-like protein aggregation may provide an enamel remineralization strategy to modify the current clinically available methods of pit and fissure sealing and shows great promise in preventing caries.

This study aims to evaluate and compare the effect of different varnishes: 3M ESPE Clinpro and Voco Profluorid and fissure sealants- 3M ESPE ClinPro and Ivoclar Vivadent Helioseal-F on the count in the saliva of children.
MATERIALS AND METHODS
Eighty children of 6-12 years of age with no incidence of caries were selected and saliva samples were collected by drooling method after oral prophylaxis. The children were divided randomly into four groups and materials were applied accordingly. Saliva samples were obtained immediately after the procedure as well as 1-month, 3-months and 6-months posttreatment. Saliva samples were inoculated on Mitis salivarius agar and colony counts of were obtained.
RESULTS
This study showed that both the varnishes and pit and fissure sealants were equally effective in reducing the salivary count in children in a time period of 6-months however pit and fissure sealants were more effective in preventing caries.
CONCLUSION
Both varnishes and pit and fissure sealants are equally effective in reducing count in saliva.

Dental sealants are an excellent means to prevent pits and fissure decay. Currently, there are multiple commercially available sealant materials. The purpose of this study was to assess the retention of glass carbomer fissure sealant and the incidence of secondary caries over a period of 24 months in comparison with a resin-based sealant.
MATERIALS AND METHODS
We included 32 children in the study, with ages between six and eight years and an average age of 6.8 years old. For each child, we sealed four permanent molars (totaling 128 teeth). The study group was divided into sub-groups. Sub-group A was represented by 64 first permanent molars which underwent dental sealing procedures with composite resin-based fissure sealant, Helioseal F™, and sub-group B was represented by 64 first permanent molars which underwent dental sealing procedures with glass carbomer cement, GCP Glass Seal™. The sealants were assessed clinically at 6, 12, 18, and 24 months.
RESULTS
The 6-month follow-up evaluation showed no statistically significant differences between the two materials neither regarding sealant retention nor new carious lesions formation ( > 0.05). At the 12-month recall, 57 molars had good retention (89.06%) from sub-group A and 44 molars (68.75%) from sub-group B; there was a statistically significant difference ( = 0.0187) between the two treatment choices only regarding material retention. At the last recall after 2 years, sub-group A had a higher number of molars with perfect sealing (47-73.43%) and 8 molars (12.5%) with new caries lesions and sub-group B had 23 (35.93%) molars with perfect sealing and 15 molars (23.44%) with new caries lesions; there was a statistically significant difference ( < 0.0001) between the two treatment choices only regarding material retention.
CONCLUSIONS
The glass carbomer retention is very inferior to the resin-based material. The glass carbomer sealant was effective in preventing new caries development, comparable with the conventional resin-based sealant.

Equine dental diseases are often underdiagnosed and their signs inadequately reported. Many horse owners have difficulties in recognizing pain-related behavioral signs and in associating them with dental pain. Our objective was to determine what type and degree of dental findings may cause behavioral signs associated with dental pain. In this cross-sectional study, dental examination was performed on 183 adult horses and cheek tooth findings were scored. Owners filled in an internet-based questionnaire including 35 questions concerning eating behavior, bit behavior, and general behavior of the horse. Descriptive statistics and logistic regression analyses were performed. Broadened or darkened fissures [odds ratio (OR) 2.4, 95% confidence interval (CI) 1.04-5.7), complicated fractures (OR 2.3, CI 1.01-5.2) and secondary dentine defects of at least the second degree (OR 3.1, CI 1.2-7.7) were associated with the expression of at least five behavioral signs in the univariable binomial logistic regression analyses. Horses with at least one of these potentially painful cheek tooth findings expressed more signs related to eating behavior, bit behavior, and general behavior than did the other horses. The results suggest that cheek tooth findings indicated by this study as being potentially painful, i.e. broadened or darkened fissures, complicated fractures and secondary dentine defects of at least the second degree, may require intervention, particularly if the horse expresses any behavioral signs that might be related to dental pain.
