Current Opinion in Ophthalmology Sep 2022To review how private equity entities generate profit for investors and ophthalmologists. (Review)
Review
PURPOSE OF REVIEW
To review how private equity entities generate profit for investors and ophthalmologists.
RECENT FINDINGS
There is a preponderance of private equity acquisitions and consolidation in ophthalmology. These private equity entities generate revenue by growth, profit improvement, and efficient use of capital structure. Physician partners sell their revenue and assets to a private equity entity while retaining a percentage of future profit. In general, a greater percentage going forward, will result in a smaller initial buyout. Partners typically receive payment in the form of cash and stock in the private equity entity, aligning incentives of both parties to grow and succeed. Junior associates and future partners typically do not benefit from the cash buyout but might have opportunity to buy shares in the private equity entity.
SUMMARY
The ophthalmology job market has changed significantly with the rise in private equity. Private equity investors profit from organic growth, economies of scale, and future revenue of ophthalmology practices. Ophthalmology partners benefit from often sizeable buyouts and potentially profitable shares in the private equity entity. Junior and future ophthalmologist may be less likely to succeed financially compared with their contemporaries. Some private equity entities will thrive and other may fail, particularly if they are unable to attract talented new ophthalmologists.

Review
PURPOSE
To discuss guidelines and ethical considerations associated with the development and prescription of treatments intended for myopia control (MC).
METHODS
Critical review of published papers and guidance documents was undertaken, with a view to carefully considering the ethical standards associated with the investigation, development, registration, marketing, prescription, and use of MC treatments.
RESULTS
The roles and responsibilities of regulatory bodies, manufacturers, academics, eye care practitioners, and patients in the use of MC treatments are explored. Particular attention is given to the ethical considerations for deciding whether to implement a MC strategy and how to implement this within a clinical trial or practice setting. Finally, the responsibilities in marketing, support, and education required to transfer required knowledge and skills to eye care practitioners and academics are discussed.
CONCLUSIONS
Undertaking MC treatment in minors creates an ethical challenge for a wide variety of stakeholders. Regulatory bodies, manufacturers, academics, and clinicians all share an ethical responsibility to ensure that the products used for MC are safe and efficacious and that patients understand the benefits and potential risks of such products. This International Myopia Institute report highlights these ethical challenges and provides stakeholders with recommendations and guidelines in the development, financial support, prescribing, and advertising of such treatments.

Review
Leadership in healthcare institutions has a direct impact on the practice of the medical and nursing professions. The respective leadership style that is cultivated in a medical institution therefore has a direct impact on the personnel and thus a direct impact on the treatment of patients, in both senses of the word. Leadership in healthcare should therefore inspire, motivate, and guide healthcare workers to use their individual and collective skills as well as the available resources in the best possible way for the benefit of patients. This is the task and obligation of all those working in the healthcare system. The German Society of Ophthalmology (DOG) has established a leadership academy for ophthalmologists to meet the requirements of the modern healthcare market for leadership qualifications.

Review
With the advance of cancer therapy in recent years, the knowledge of the mechanisms involved in this disease has increased, which has meant an increase in the quality of life and survival of patients with tumor pathologies previously considered incurable or refractory to treatment. The number of drugs used has increased exponentially in number, and although the implicit toxicity is lower than that of conventional antineoplastic therapy, they lead to the appearance of new associated adverse effects that the ophthalmologist must recognize and manage.

OBJECTIVE
The aim: To consider the organization of ophthalmological care in Ukraine for cataract and glaucoma and find out whether it is appropriate to implement advanced good practices of reference countries for Ukraine.
PATIENTS AND METHODS
Materials and methods: The method of desk review was used, supported by secondary analysis of data, in particular legislative acts. The research included conducting expert interviews with ophthalmologist of the public and private sectors, heads of public healthcare institutions, and management of the National Health Service of Ukraine. We also used materials on good practices from partners within the project ID 22120107 supported by Visegrád Fund.
RESULTS
Results: Since the burden of ophthalmological pathologies is increasing and processes of reforming the health care system are taking place, changes in the organization and financing of ophthalmological services are taking place. Within the framework of the partner project, "Access to healthcare services in the context of financing mechanisms. The case of ophthalmology" identified good practices in the organization of ophthalmological care in the context of improving access to services and improving quality. The results of interviews with key stakeholders led to the fact that the respondents generally support all the good practices proposed by the partner countries and indicate their arguments why the proposed practices are (not)appropriate to implement in Ukraine.
CONCLUSION
Conclusions: The organization and financing of healthcare in Ukraine still require the study and implementation of good practices so that patients can have access to quality services and treatment.

This article presents a creative biography of professor A.A. Kryukov - a prominent Russian ophthalmologist, compiled on the basis of previously unpublished archival information and analysis of literary sources. The work notes constant attention and trust of his teacher M.M. Voinov, associate professor of the Medical Faculty of Moscow University; describes the main directions of scientific research of A.A. Kryukov, emphasizes the social significance of his work, the long-lasting high demand for his «Textbook of Eye Diseases». A.A. Kryukov prioritized development and implementation of iridectomy through a scleral incision in the ophthalmic practice. He was interested in developing ties with European colleagues, constantly assisted with dissemination of scientific information, cooperated with domestic and foreign mass media. He had an initiative role in the establishment and development of the Moscow Ophthalmological Circle. The article also amends the mistake of modern researchers who present A.A. Kryukov as a pioneer of local anesthesia in ophthalmology.
