Clinical Trial Comparative Study Randomized Controlled Trial
The effect of azathioprine in patients with active Crohn's disease has been evaluated in a double-blind cross-over trial. Fifteen patients were treated with both azathioprine and placebo: only two improved while on azathioprine, six got worse, and in seven there was no change. The drug seemed to be the cause of deterioration in two of the patients. Although the drug may be of some value in a few patients, it should be used with caution and only when other measures have failed.

Cirrhosis resulting from autoimmune hepatitis is associated with an increased risk of hepatocellular carcinoma. A common treatment for autoimmune hepatitis, azathioprine, is also associated with the development of many other cancers, predominantly lymphomas. The strongest association is seen for post-transplant lymphoma and hepatosplenic T-cell lymphoma in Crohn's disease and ulcerative colitis patients; there is also an association with a variety of cutaneous malignancies. A relationship between azathioprine and sarcoma has not been demonstrated, though there have been sporadic case reports. We report here the development of leiomyosarcoma in a patient who was treated with azathioprine for autoimmune hepatitis without cirrhosis.

Azathioprine may be tried for treatment of severe myasthenia gravis, in a dose of 2 mg/kg BW. With careful monitoring the side reactions may be controlled. The therapeutic response will occur after a time delay of 4-8 months. About one-third of patients with type II MG (severe, late onset, HLA B8-) will proceed to a complete but azathioprine-dependent remission, and the remaining two-thirds to a marked improvement. In type I MG (early onset, HLA B8+) azathioprine is less effective but will help keep down the need for corticosteroids or plasmapheresis.

Therapy with azathioprine and cyclophosphamide is described as reducing disease activity in rheumatoid arthritis. The well recognized untoward effects are tractable save for the definite augmentation of carcinogenesis, a measurable risk that is regarded as acceptable in treating rheumatoid arthritis with azathioprine and rheumatoid arteritis with cyclophosphamide.
