Review
Newborns with congenital diaphragmatic hernia (CDH) present challenges for the neonatal intensive-care nurse. Although CDH has been a known condition for almost 200 years, the treatment strategy for newborns with CDH has changed during the past decade. Despite these improvements, the mortality rate for this condition remains high. An understanding of the anatomical basis and new treatment modalities for this condition will prepare nurses to care for these newborns.

The physical signs of these and other reported cases are discussed, as are methods of diagnosis. Definitive diagnosis is best arrived at by exploratory laparotomy.

"Acquired" congenital diaphragmatic hernia (ACDH), has been defined as delayed or late appearance of a congenital diaphragmatic hernia after a documented time periol of postnatal life with no evidence of herniation. Three new cases are presented. This diagnosis has been characterized on the basis of a review of these cases and 14 additional patients from the literature. A classification based upon timing of herniation and state of pulmonary development is presented.

The presentation of a congenital diaphragmatic hernia (CDH) beyond infancy is rare, as it is a common cause for severe respiratory distress in the newborn. In contrast to the newborn period, delayed diagnosis of right-sided congenital diaphragmatic hernia (RCDH) is known as it causes less severe symptoms than left-sided CDH. A case of an 8-month-old girl is reported. Chest x-ray and ultrasound examination of the diaphragm for symptoms of pneumonia revealed an RCDH. Thoracoscopic repair could be performed. The further course was uneventful. Thoracoscopic repair of a RCDH proved in this case to be feasible and safe beyond the newborn period. It avoided the morbidity of a major thoracotomy or laparotomy and provided an excellent cosmetic result. To our knowledge, this is the first report of thoracoscopic repair of RCDH.
