Journal of Computer Assisted Tomography 1994We determined the MR and CT appearance of ovarian dysgerminoma and correlated our findings with the pathologic features.
OBJECTIVE
We determined the MR and CT appearance of ovarian dysgerminoma and correlated our findings with the pathologic features.
MATERIALS AND METHODS
CT and MR findings of three patients with ovarian dysgerminoma were retrospectively reviewed and compared with the pathologic findings.
RESULTS
The tumors were divided into lobules by septa that were hypointense or isointense on T2-weighted imaging and showed marked enhancement on Gd-diethylene triamine pentaacetic acid enhanced T1-weighted imaging and contrast-enhanced CT. These septa corresponded to fibrovascular bundles at histologic examination.
CONCLUSION
CT and MR findings characteristic of fibrovascular septa within solid ovarian masses should raise the possibility of dysgerminomas.

BACKGROUND
Imaging findings have a prominent role in early and correct identification of ovarian dysgerminoma, the most common ovarian malignant germ cell tumor (OMGCT). Despite Computed Tomography (CT) is widely used, Magnetic Resonance Imaging (MRI) has proved to be superior in adnexal masses characterization. Limited data and small series are available concerning MRI aspects of dysgerminoma.
CASE PRESENTATION
From January 2012 to December 2018, a database of solid ovarian masses was retrospectively reviewed. Eight patients with histologically proven pure ovarian dysgerminoma and complete imaging available were identified and analyzed. Imaging findings were evaluated separately by two radiologists expert in female genito-urinary MRI.
CONCLUSIONS
MRI findings of a lobulated, purely solid, encapsulated mass with hyper-intensity of lobules and hypo-intensity of septa on T2w images contribute to differentiate dysgerminomas from other ovarian neoplasms.

Review
Lactic dehydrogenase (LDH) is a glycolytic enzyme that may be elevated in the serum of patients with gonadal and extragonadal germinomas. In this report, a case of unilateral ovarian pure dysgerminoma with remarkably elevated levels of serum LDH is presented. After complete excision of the tumor, serum LDH levels promptly returned to normal levels. Although an electrophoretic pattern of serum LDH isoenzymes was within normal limits pre-operatively, an increase in LDH-1 and 2 was found 1 week after operation. Seventeen additional examples of ovarian dysgerminomas with elevated serum LDH levels have been reported in the English literature including five cases who had high levels of two fast fractions of LDH. These data suggest that serum LDH levels and its isoenzymes pattern are useful tumor markers for diagnosis and post-therapy surveillance in patients with ovarian dysgerminomas.

Dysgerminomas account for approximately one third of all malignant ovarian germ cell tumors (tumors arising from ovarian germinal elements) and are the most common ovarian malignancy detected during pregnancy. They are the only germ cell malignancy with a significant rate of bilateral ovarian involvement that is 15-20 percent. They have a variable gross appearance, but in general are solid, pink to tan to cream colored lobulated masses. They have the best prognosis of all malignant ovarian germ cell tumor variants. Two thirds are stage I at diagnosis, and prognosis is excellent even for those with advanced disease due to exquisite tumor chemosensitivity. The 5 year disease specific survival rate approximates 99 percent. This is a case report of a huge ovarian dysgerminoma in a young unmarried lady that was quite asymptomatic. She underwent laparotomy with right ovarian cystectomy.

Nine cases of dysgerminoma are reported. Five patients developed recurrence of disease in either the pelvis or metastatic sites including the lung and brain. The metastatic disease was controlled by a radiation dose of 2,000 to 3,000 rads during three to four weeks in four of the patients. The excellent response of dysgerminoma to radiation warrants an optimistic prognosis and vigilant follow-up since metastatic disease can be controlled by moderate radiation doses.

METHODS
French patients (≤18years) treated for dysgerminoma between 1985 and 2005 in TGM-85, 90, 95 protocols were included. Treatment was based on primary unilateral oophorectomy followed by prophylactic lymph node irradiation (1985-1998) or a wait-and-see strategy (1998-2005) for localised completely resected tumours (pS1) or by platinum-based chemotherapy for advanced diseases.
RESULTS
Forty-eight patients (median age 12.8 years) were included. Six patients had gonadal dysgenesis. Two had bilateral dysgerminoma. Twenty-eight patients had loco-regional dissemination, seven with para-aortic lymph nodes. None had distant metastases. Primary surgery was performed in 47/48 patients. Among the 15 patients with pS1 tumour: seven did not receive adjuvant treatment, six had lymph node irradiation and two received chemotherapy. Among the 32 patients with advanced tumour, 31 received cisplatinum-based (n = 25) or carboplatin-based (n = 8) regimen with lymph node irradiation for one of them and one did not receive adjuvant treatment. With a median follow-up of 14 years, all patients are alive in complete remission. Five events occurred: 2 contralateral dysgerminomas, 1 peritoneal relapse and 2 second neoplasms (teratoma and melanoma). Bilateral oophorectomy was necessary for 12 patients. Desire of pregnancy was expressed for 17/36 patients with unilateral oophorectomy, which succeeded in 13 cases (5 medically assisted). 2/17 had ovarian failure. The renal function was normal in 24/25 evaluated patients treated with platinum, ifosfamide or irradiation. The hearing function was evaluated on 17/36 patients treated with platinum: 12 Brock grade-0, 3 brock grade-1 and 2 grade-4.
CONCLUSION
Dysgerminoma has an excellent prognosis even in advanced cases with conservative surgery and platinum-based chemotherapy. However the disease and/or treatment resulted in a high rate of bilateral oophorectomies and a significant impact on future fertility.

Dysgerminomas of the ovary are rare germ cell tumors. They are more common at puberty. Surgery followed by irradiation has resulted in excellent cure rates in early stages of this disease. This paper is a review of six patients with a diagnosis of dysgerminoma who were treated at the Department of Radiation Oncology, University of Louisville. The controversy on the use of irradiation in Stage Ia tumor is discussed in detail. A review of the literature is also done in this paper.

From 1938 to 1977, 21 patients with pure dysgerminoma were treated at Memorial and James Ewing Hospitals, now Memorial Sloan-Kettering Cancer Center. Twelve patients were considered to have their initial therapy begun or completed at our institution. Eleven (91.7%) of 12 patients were found free of disease from 2 to 38 years (median 6 years). Nine patients were referred with recurrent disease. Only 4 (45%) could be salvaged 2 to 21 years (median 9 years). Because of the lower survival rate in the treatment of recurrent disease in this series, it is difficult to advocate overly conservative therapy initially in all early lesions. The survival rate depends on the original size of the tumor, initial adequate staging and appropriate therapy based on the extent of disease. Recommendations for a treatment protocol are proposed.
