We analyzed five cases of pure dysgerminoma following the same schedule for the treatment, which were studied during the period of 1971 to 1983. The age of patients during the period of histological examination ranged from 15 years and 6 months to 28 years. All five patients had already had their "first menstruation". The above cases examined according to the clinical findings, the symptoms, the stage of the disease; the sort of treatment the place, the period and kind of therapy in case of reappearance of the disease and final survival.

The authors report a case of stage 1 ovarian dysgerminoma (OD) in a young woman aged 23 who subsequently underwent conservative surgery. A review of the literature shows that in 80% of cases this neoplasia is present at stage 1A and 85% of patients are under 30, with a 5-year survival rate of 95%. OD treatment should therefore be personalized and, although it continues to be controversial, most authors agree that in young women in whom the disease is limited to one ovary, without lymph node metastases, treatment can be restricted to monolateral adnexectomy, thus assuring the patient's future fertility.

Dysgerminomas comprise approximately 2-5% of all ovarian malignancies and mostly affect young adolescent women. Primary comprehensive surgery and adjuvant chemotherapy consisting of bleomycin, etoposide, and cisplatin (BEP) are the current recommended treatment options, the latter reserved for advanced stages (FIGO II-IV). We report two patients aged 20 and 26 years who presented with an initial FIGO stage IA, but inadequately assessed. Both were subsequently diagnosed with recurrent malignant dysgerminoma and para-aortic lymph node metastasis. Neither had received comprehensive staging at initial surgery nor adjuvant radio or chemotherapy. Both needed extensive surgery and multiagent chemotherapy for survival and belong to the small percentage of FIGO IA dysgerminoma patients showing a relapse. Comprehensive initial surgery including systematic para-aortic lymphadenectomy and adjuvant chemotherapy at tertiary referral centers is needed to minimize the treatment burden.

Two new cases of primary mediastinal seminoma are presented and 126 cases from the literature are analysed. Radiotherapy, alone or combined with surgery, is thought by some authors to be the treatment of choice and the role of chemotherapy is uncertain. We propose a classification by which it is possible to compare various therapeutic approaches and to establish their value.

Four patients with stage IA-IIIC dysgerminomas were found to have preoperative elevations of serum lactic dehydrogenase (LDH) 1.8-51.5 times that of the upper limits of normal control values, in association with minor elevations of liver function tests (alkaline phosphatase, serum glutamic oxaloacetic transaminase, and serum glutamic pyruvic transaminase) up to 2.5 times normal control limits. Recognition of the association between this serum enzyme profile and dysgerminoma resulted in proper intraoperative staging and prompt initiation of chemotherapy in two patients. Failure to recognize the significance of the serum enzyme profile resulted in inadequate intraoperative staging and reoperation in one of the remaining patients. Serum LDH levels correlated with tumor size and stage of disease. Maximum tumor diameters in this series varied from 15-20 cm. A normal LDH level was present in an intraoperative serum sample obtained from a stage IA patient whose maximum tumor diameter was 8 cm. Selective elevation of LDH-1 and LDH-2 isoenzymes may provide additional evidence suggesting the presence of a dysgerminoma.

Germline mutations in the BRCA1 tumor suppressor gene are associated with increased risk for the development of ovarian cancer. All such cancers thus far reported have been of the epithelial histologic type. We identified an ovarian dysgerminoma in a 16-year-old woman (proband) with a family history of ovarian cancer during a review of histopathologic characteristics of ovarian cancers from women enrolled in the Gilda Radner Familial Ovarian Cancer Registry. Mutation analysis of DNA from this patient's peripheral blood leukocytes revealed a germline BRCA1 mutation (3312insG). The mutation was also present in the mother with breast cancer, a maternal aunt and a distant cousin with ovarian cancer, and a maternal grandfather and an uncle with skin cancer. The development of the proband's dysgerminoma may be unrelated to her germline BRCA1 mutation. Alternatively, such dysgerminomas may be caused by BRCA1 mutations, but occur so infrequently compared with epithelial cancers that they are seldom identified. Analysis of a larger series of ovarian germ cell tumors may resolve this question.

lactic dehydrogenase (LDH) is a glycolytic enzyme that may be elevated in the serum of patients with gonadal and extragonadal germinomas. In this report, the authors describe a young woman with a unilateral ovarian pure germinoma (dysgerminoma) who had greater than a sixfold elevation of serum LDH levels with increases in LDH isoenzymes 1, 2, and 3. Two weeks after operation, the serum LDH level had returned to normal. Four additional examples of ovarian germinoma with elevated serum LDH levels have been recorded in the literature. The germinoma cells are the likely source of the elevated LDH activity in such cases. Lactic dehydrogenase and its isoenzymes may serve as a useful tumor marker for diagnosis and posttherapy surveillance in patients with germinomas (dysgerminomas, seminomas). The association of elevated serum LDH in patients with other neoplasms is also reviewed.
