Although genital exhibitionism has traditionally been almost exclusively associated with males, similar behavior may occur in certain women with a particular set of life circumstances and precipitants. A case report is presented of a woman whose emotional deprivation led to feelings of rejection and the inability to develop empathic relationships. Her self-esteem relied on success in her profession, and when she lost her position, she sought validation through exhibitionism. Despite the widely held view that genital exhibitionism producing arousal and stimulation in the subject occurs only in men, this case demonstrates that such phenomena may exist in women as well.

I reviewed the empirical literature for 1980-2008 on exhibitionism, voyeurism, and frotteurism for the American Psychiatric Association's Sexual and Gender Identity Disorders Work Group in preparation for the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-V). Very limited empirical support was found for major changes of the current DSM-IV-TR criteria sets for these paraphilias. Some of the criticism of current criteria and the balancing of false negatives and false positive diagnoses are examined. The report concludes with suggestions for possible diagnostic criteria changes for the DSM-V.

Exhibitionistic Disorder, one of the paraphilic disorders, is a disease with an unknown etiology and causes significant distress and loss of function in the patient's life. Serotonergic antidepressants are generally preferred in the treatment of this Disorder. However, in this case, we report a patient who did not respond to serotonergic antidepressants but bupropion, an antidepressant with dopaminergic and noradrenergic activity. Therefore, bupropion should be considered a medical treatment alternative in case serotonergic antidepressants do not work efficiently in the treatment of Exhibitionism.
