BACKGROUND
The application of fractional Q-switched ruby laser (FQSRL) or intense pulsed light (IPL) on Café-au-lait macule (CALM) is rational and the data are lacking.
OBJECTIVE
To evaluate the efficacy and safety of FQSRL and IPL in CALM.
METHODS
The patients with CALM who were treated with FQSRL or IPL were retrospectively observed from April 2016 to April 2019. The laser/light treatments were conducted at an interval of 3-4 weeks.
RESULTS
For FQSRL ( = 67), 88.23%, 95.46%, 100% patients achieved >50% improvement by three sessions, four sessions, and more than four sessions of treatment, respectively. A better and better efficacy was shown with the increasing number of sessions ( = 89.51, < .01). For IPL ( = 54), 45% and 87.5% achieved >50% improvement by three sessions and more than four sessions of treatments, respectively. More than four sessions achieved better efficacy than less sessions ( < .01). Under various time-points, FQSRL presented more favorable responses than IPL ( < .05). All the adverse effects were tolerable and acceptable.
CONCLUSIONS
FQSRL or IPL would be an alternative and safe modality for CAML in Chinese patients.

A review of the literature is done concerning labial melanotic macule. Several aspects are studied such as definition, clinical significance and histopathology. A new case is added, located in the lower lip. Several observations are done and conclusions are drawn. Emphasis is given to the need of determining the etiology of the lesion.

The clinical and histologic spectrum of melanotic macule of the nail unit is examined and the differences in the clinical appearance of longitudinal melanochychia caused by melanotic macule and by other kinds of proliferations of melanocytes are assessed. We observed that the clinical appearance of the pigmented band was of little help in establishing the underlying basic pathologic process. This underscores the importance of obtaining a biopsy of the nail matrix in patients who present with solitary longitudinal melanonychia.

A 16-year-old boy presented with a number of asymptomatic pigmented macules on the volar aspect of his index fingers. Dermoscopy of each macule revealed a parallel ridge pattern of homogenous reddish-brown pigment. We propose that these lesions were induced by repetitive trauma from a Sony PlayStation 3 (Sony Corporation, Tokyo, Japan) vibration feedback controller. The lesions completely resolved following abstinence from gaming over a number of weeks. Although the parallel ridge pattern is typically the hallmark for early acral lentiginous melanoma, it may be observed in a limited number of benign entities, including subcorneal haematoma.

The labial melanotic macule has an increased number of dendritic melanocytes along the basal layer. Numerous counterparts of the labial melanotic macule occur on the other mucosal and cutaneous surfaces. Most melanoacanthomas of the lip show a similar number of melanocytes along the junctional zone. Melanoacanthoma of the lip differs histologically from the labial melanotic macule by the presence of intraepithelial dendritic melanocytes. It also has histologic counterparts that occur on other oral mucosal surfaces. Primary acquired melanosis of the conjunctiva is biologically different from the other mucosal melanoses because it frequently progresses to malignant melanoma. All mucosal melanoses must be taken seriously by the physician, because it is usually not possible to exclude malignant melanoma on clinical inspection alone. A biopsy is always indicated. The labial melanotic macule and melanoacanthoma demonstrate histologic features strikingly different from those of melanoma. Although intraepithelial dendritic melanocytes are seen in melanoacanthoma, they are mostly present in the lower portions of the epithelium, and there is no full-thickness haphazard dispersion of atypical single and nested melanocytes. Along the basal layer in melanotic macules and melanoacanthomas of the lip, melanocytes are arranged as solitary units without a tendency to form abnormal nests or become confluent, in contrast to the picture in melanoma. Melanocytes in this zone show no pleomorphism, in contradistinction to the often bizarre, atypical melanocytes seen in melanoma.

A lentigo is a small pigmented macule with a sharply circumscribed border. There are multiple clinical and etiologic forms. Lentigines are often initially identified shortly after birth, although they may appear later in childhood. Certain varieties are associated with systemic abnormalities. Histologic findings include epidermal hyperplasia with increased pigmentation of the basal layer.
