Meckel's diverticulum is present in 2% of the population. Duplications of the alimentary tract are rare congenital malformations. The small bowel is the commonest site of alimentary tract duplication. They mimic other surgical diseases and may result in significant morbidity if left untreated. Pre-operative diagnosis of Meckel's diverticulum is difficult. A case of duplication of Meckel's diverticulum is reported here.

Meckel's diverticulum is a common anomaly of the small intestine and occasionally presents as obscure gastrointestinal hemorrhage. Before operation, it is difficult to diagnose by imaging, especially in adults. Conventional abdominal computed tomography and endoscopy have limitations for the diagnosis of Meckel's diverticulum. Diagnostic methods in patients with small bowel lesions include enteroclysis, angiography, push enteroscopy, and capsule endoscopy; however, all of these techniques have low diagnostic yields to detect Meckel's diverticulum. Recently, computed tomographic enterography has been reliable in assessing small bowel disease. We present 3 cases of Meckel's diverticulum with bleeding in adults who were diagnosed by computed tomographic enterography. The bleeding source was not found in the total colonoscopy, and Tc-99m pertechnetate scans were negative in these patients. However, outpouching structures of the distal ileum with enhancement were detected by computed tomographic enterography. All patients underwent small bowel segmental resection. Meckel's diverticulum was confirmed by histopathology of the resected ileum segment, and the type of heterotopic tissue was gastric mucosa.

Meckel's diverticulum (MD) is usually a simple tubular-shaped diverticulum. We describe a MD with multiple complex terminal sprouts in a child found incidentally during an appendectomy for appendicitis. The MD was resected, and the child recovered well. MD may show multiple sprouts. There was no additional clinical consequence in this child with the malformed MD.

This is a study of Meckel's diverticulum in adults who had undergone surgery in the hospital. The complications of Meckel's diverticulum are compared with that of the ones mentioned in the literature. A comparative study of the clinical features and the operative findings in case of Meckel's diverticulum with ectopic tissue is also discussed. The controversy of Meckel's diverticulectomy has also been dealt with at large.

Meckel diverticulum is one of the causes of gastrointestinal bleeding; however, preoperative conclusive diagnosis is very difficult even with the use of various radiologic modalities. The development of double-balloon enteroscopy enables us to diagnose various bleeding source, including Meckel diverticulum, in the small intestine. We herein report a case of bleeding Meckel diverticulum, diagnosed by double-balloon enteroscopy and treated successfully by laparoscopic surgery, and review of the literature.
