The outlook for patients with Hodgkin's disease has improved dramatically over the past 20 years. The question is no longer whether cure is possible, but rather, how can cure be best achieved. With better understanding of the biology of Hodgkin's disease and with continued evolution of treatment approaches, the goal of curing all patients with Hodgkin's disease is clearly within reach. This article provides a summary of current concepts in the biology and management of Hodgkin's disease. Staging, treatment options, and complications of therapy are discussed.

A case of composite lymphoma in a young female is described. The patient was found to have lymphocyte-predominant Hodgkin's disease at presentation, but subsequent staging laparotomy revealed coexistent but distinct Hodgkin's disease and diffuse large cell lymphoma in the spleen. Possible explanations for this rare occurrence are discussed, and the literature is reviewed.

After an historical review of the progressive improvement of Radiotherapy of Hodgkin's Disease, the authors present the actual management concepts with special reference to prognostic factors. So, the actual strategy of Hodgkin's Disease aims above all not to lower the "cure" values lately reached but to reduce the iatrogenic pathology trying not to treat to much the good prognosis subgroups. These are susceptible of a less aggressive therapy by the knowledge of the relation cost/benefit of the intensive radiotherapy and the several multidrug regimens.

Pulmonary toxicity is a well recognised side effect of anticancer agents particularly bleomycin, cyclophosphamide, methotrexate, and busulphan. In contrast this problem has been infrequently reported following MOPP (nitrogen mustard, vincristine, prednisolone, procarbazine) chemotherapy for Hodgkin's disease and has been attributed principally to the procarbazine. We report a further instance of MOPP associated pulmonary toxicity. This case and a review of previously published cases indicate that MOPP chemotherapy may be associated with the development of permanent lung damage as well as an acute reversible hypersensitivity lung disease.

The purpose of this retrospective study was to evaluate the efficacy and toxicity of the MOPP chemotherapy protocol (mechlorethamine, vincristine, procarbazine, and prednisone) as a rescue regimen in dogs with lymphoma. One hundred seventeen dogs that had resistance to previously administered chemotherapy were evaluated. Before treatment with MOPP, all dogs received a median of 6 chemotherapy drugs for a median duration of 213 days. Thirty-one percent (36 of 117) had a complete response (CR) to MOPP for a median of 63 days, and 34% (40 of 117) had a partial response (PR) for a median of 47 days. Sixteen percent (19 of 117) had stable disease (SD) for a median of 33 days. Predictors for response to MOPP were not identified. Gastrointestinal (GI) toxicity occurred in 28% (33 of 117) of the dogs, and 13% (15 dogs) required hospitalization. Five dogs developed septicemia, and 2 died as a result. MOPP was an effective treatment for dogs with resistant lymphoma and was well tolerated by the majority of affected dogs.

Hodgkin's disease (HD) appears to occur more frequently in people with HIV and within this population HD demonstrates a number of differences. In people with HIV, Epstein-Barr virus (EBV) is found more frequently in Reed-Sternberg cells and mixed cellularity and lymphocyte depleted histological subtypes predominate. Patients with HIV present with more advanced stage HD often with extranodal disease. The prognostic variables for HD in patients with HIV relate to the HIV rather than the HD. In people with HIV the response rates are lower, the relapse rates higher, the infectious complications more frequent and the overall survival worse than for HD in the general population.

The improved survival rates among patients with Hodgkin's lymphoma over the past few decades have come with increased incidence of second malignancies. One of the major concerns among female survivors is the significantly elevated risk of breast cancer that appears with extended follow-up. In this review, we include the published literature regarding the risk of breast cancer after irradiation for Hodgkin's lymphoma. We also present the possible long-term surveillance strategies and the optimal time to start screening these women. This could potentially help in early detection of secondary breast cancers and consequently improve outcomes. Furthermore, because of prior radiotherapy, the management of the breast cancer among this unique population has been controversial. We discuss the characteristics of breast cancer that occurs after Hodgkin's lymphoma and also treatment options that could be implemented.

During the past 2 decades, major medical advances have resulted in dramatic improvements in cure rates for the majority of children with cancer, including those with Hodgkin's disease. Long-term data from different Institutions reveal actuarial 5, 10 and 15 year survival rates of 90, 80 and 70% and corresponding freedom from relapse rates of 75, 66 and 66% respectively. The prices that are paid to achieve these results are high in term of growth and development, sterility and second malignant tumor. The present challenge in the treatment of children with Hodgkin's disease is to delineate a therapeutic program which will insure continuation of the excellent survival and disease-free survival which are now being achieved while minimizing the risks of complications from treatment. At present it appears that the combination of low dose irradiation and chemotherapy (alternating non-cross-resistant drug combinations: MOPP/ABVD) is successful in affording optimistic cure rate with minimal morbidity. However, there are many questions which require a specific answer in the future: to define the risk/benefit ratio of therapy and to establish the role of surgical staging in the pediatric population.
