Review
Although known as a separate group of organisms since 1898, the isolation in 1937 of a human genital mycoplasma represented the first observation of the association of this group of organisms with humans, a discovery that provided the impetus for extensive investigations of the interactions of mycoplasmas with the human host. These studies showed that mycoplasmas were frequent inhabitants of the mucous membranes of the urogenital and upper respiratory tracts. In early studies of human mycoplasmas, they were not speciated but were assigned to this group of organisms almost exclusively on the basis of cultural characteristics. However, differentiation between groups of human genital and oral mycoplasmas based on cultural, morphologic, biochemical, and serologic properties was achieved in 1953. A few years later formal proposals for the establishment of three human species of Mycoplasma (including Mycoplasma hominis) were made, along with suggestions for a new classification and nomenclature of the mycoplasmas. Serologic, genetic, and other data clearly show that M. hominis does constitute a relatively heterogeneous group of organisms. However, although this heterogeneity has practical implications for identification of isolates of M. hominis and for demonstration of antibodies to M. hominis, further studies are necessary to justify formal recognition of a taxonomic subdivision of M. hominis into distinct serovars. Investigations of the role of M. hominis as a potential human pathogen date back to the very first years of its discovery.(ABSTRACT TRUNCATED AT 250 WORDS)

Mycoplasma genitalium (Mge) and Mycoplasma pneumoniae (Mpn) are two human pathogens associated with urogenital and respiratory tract infections, respectively. The recent elucidation of the tridimensional structure of their major cytoadhesins by X-ray crystallography and cryo-electron microscopy/tomography, has provided important insights regarding the mechanics of infection and evasion of immune surveillance.

Mycoplasma pneumoniae and M. gallisepticum possess binding sites of protein nature which mediate attachment to neuraminidase-sensitive regions on both respiratory epithelium and red blood cells. The binding sites of these organisms are similar though not identical. Several approaches were applied for the isolation of the binding sites. Of these, the use of affinity chromatography yield the least complex protein fraction. We have recently been using sialoglycopeptides as the ligands in affinity chromatography. The availability of monoclonal antibodies which inhibit the attachment of M. pneumoniae to host cells should provide a very specific tool for the isolation of the attachment moiety. It should be mentioned that not all mycoplasmas adhere to host cells via sialic acid specific receptors, and other approaches should be developed to study these mycoplasmas' attachment moieties.

Mycoplasma contaminations have a multitude of effects on cultured cell lines that can potentially influence the results of experiments and pollute bioactive substances used in human medicine. The elimination of mycoplasma contaminations in cell cultures has become a practical alternative to discarding and re-establishing important or irreplaceable cell lines. Different quinolones, tetracyclins, and macrolides shown to have strong antimycoplasma properties are employed for the decontamination. We provide detailed descriptions to assure eradication of mycoplasma, to prevent formation of resistant mycoplasma strains, and to cure heavily contaminated and damaged cells. To date, no consistent and permanent alterations that affect the eukaryotic cells during or after the treatment have been detected.

The article presents current views on urogenital diseases caused by M. hominis and U. urealyticum. Particular attention is paid to concurrent (co-occurring) urogenital infections. The review reports the data from epidemiological studies and outlines recent Russian and international guidelines and consensuses on managing patients with urogenital inflammatory diseases. The importance of adequate diagnosis and rational therapy of urogenital infections is discussed.
