Four patients with paraproteinemia and scleredema were studied. Histologic features included marked thickening and fibrosis of the dermis and subcutis. Variable amounts of mucin deposits were detected in the interfibrillar spaces. Serum from one patient significantly stimulated collagen production in normal skin fibroblast cultures, whereas serum from another patient stimulated collagen production in autologous cell cultures. Moreover, serum from one patient stimulated the [35S]sulfate incorporation into the fibroblasts. Circulating serum factors, possibly related to the paraprotein, may enhance the synthesis of extracellular macromolecules by dermal fibroblasts in these patients, thus providing a mechanism for dermal fibrosis.

Review
A case of hairy cell leukemia and IgG paraproteinemia is described. Peripheral blood surface marker analysis, serum paraprotein levels and immunoperoxidase stains of bone marrow sections at diagnosis and after 7 months of interferon treatment suggested the existence of two unrelated pathological B cell clones: one composed of malignant hairy cells and the other secreting the paraprotein. Previously reported cases of hairy cell leukemia with paraproteins are reviewed and our patient's contribution to the understanding of this association is stressed.

Transplantation experiments in the C57BL/KaLwRij mouse model of idiopathic paraproteinemia (IP) showed that an IP-producing clone can be further propagated in young, lethally irradiated mice and also equally as well in nonirradiated recipients by a bone marrow and/or spleen cell transfer. The latency period before the original paraprotein was detected in the sera of recipients varied in different experiments between 1 and 9 months after transplantation. With subsequent transplantations, the "take" frequency gradually decreased. Propagation of IP for three to four generations seems to be the final limit. In comparison to age-matched seems to be the final limit. In comparison to age-matched control groups, no substantial influence of the transplanted IP on the survival of the recipients was observed. In contrast, transplantation of cells from mice with a B cell lymphoma or a myeloma led to continuous propagation of the malignancy, with a high "take" frequency, progressive development of the paraproteinemia, and a shortened survival time of the recipients. These findings indicate that IP represents in its final stage in the aging C57BL mice an intrinsic cellular defect within the affected B cell clone, which is, however, different from that found in B cell malignancies.

Mantle cell lymphoma (MCL) is most frequently diagnosed by the routine histological (HP) and immunohistochemical (IH) examination. Herein we present a case of 47-yr-old female with general lymphadenopathy and leukemic blood picture. She was initially diagnosed with marginal zone lymphoma (MZL). This diagnosis was based not only on the HP/IH examination but also on the presence of IgM paraproteinemia. Flow cytometry (FCM) examination was helpful in making of the final diagnosis of MCL. Fluorescence in situ hybridization and cyclin D1 mRNA detection by RT-PCR additionally confirmed the MCL diagnosis. The IgM paraproteinemia is rare in MCL, although is a common feature of lymphoplasmacytoid lymphomas (LPL) and is considered a major characteristic of Waldenström's macroglobulinemia (WM).
