A history of literature and views existing within the COC (Cultural and Recreational Center), the major Dutch organization of homosexual men and women, regarding pedophilia and its relationship to homosexuality are discussed, beginning with definite separation between the pedophile and homosexual identities and ending with an abolishment of oppression towards pedophilia, for which the COC is in part responsible. The article argues that the homosexual identify is by no means a "constant" but a fluid identity, based on societal views and conditions. By accepting pedophilia, the COC hopefully will broaden the idea of the gay identity.

The present paper illustrates the approach of a forensic psychiatric expert witness regarding the assessment of pedophilia. In a first step it is inevitable to differentiate if the defendant is suffering from pedophilia or if the alleged crime might have been committed because of other motivations (antisociality, sexual activity as redirection, impulsivity). A sound diagnostic assessment is indispendable for this task. In a second step the level of severity needs to be gauged in order to clarify whether the requirement of the entry criteria of §§ 20, 21 of the German penal code are fulfilled. In a third step, significant impairments of self-control mechanisms need to be elucidated. The present article reviews indicators of such impairments regarding pedophilia. With respect to a mandatory treatment order (§ 63 German penal code) or preventive detention (§ 66 German penal code) the legal prognosis of the defendant needs to be considered. The present paper gives an overview of the current state of risk assessment research and discusses the transfer to an individual prognosis critically.

In the context of a search for testable etiological theories of pedophilia, the relationship of pedophilia to partner sex preference was investigated. The proportional prevalences of gynephilia and androphilia were compared with the proportional prevalences of sexual offenders victimizing female children and of such offenders against male children. Since pedophilia either does not exist at all in women, or is extremely rare, only men were included in the study. We derived the proportional prevalence of androphilia from a review of the main pertinent studies, including Gebhard's reassessment of the study by Kinsey et al. particularly of the section on gynephilia vs. androphilia. The numbers of heterosexual vs. homosexual offenders against children were derived from the studies by Mohr et al., by Gebhard et al., and from a group of 457 sex offenders against children seen in the course of several years at the Department of Behavioural Sexology of the Clarke Institute of Psychiatry. There was a large difference between the proportion of prevalences of heterosexual vs. homosexual offenders against children on the one hand and the proportional prevalences of gynephilia vs. androphilia, on the other. This difference suggests that the development of erotically preferred partner sex and partner age are not independent of each other and that in pedophilia, the development of heterosexuality or homosexuality is brought about by factors different from those operative in the development of androphilia or gynephilia.

Pedophilia is a disorder of sexual preference that increases the risk for committing sexual offenses against children. Consequently, pedophilia is not only relevant in psychiatric therapy and prognostics, but also greatly influences the public attitude towards criminality. Public opinion seems to equate pedophilia with child sexual abuse and vice versa which leads to stigmatization of patients and may impede treatment. The present paper provides information on recent studies on the potential origins of the disorder and introduces new diagnostic methods. Moreover, the article presents estimates on the prevalence of pedophilic sexual interest.

We investigated the sensitivity of our phallometric test for pedophilia (and hebephilia). An initial sample of subjects included 47 men accused of sexual offenses against minors and 26 control subjects-men accused of offenses against adult women (exhibitionism, rape, or sexually sadistic activity). A second sample included 107 offenders against minors and 30 control subjects. In both samples, the offenders against minors were further classified according to the targets of their sexual offenses (girls, boys, or both) and according to the extent to which they admitted an erotic preference for the immature physique. Computerized diagnostic rules were developed with the first sample and cross-validated with the second. The sensitivity of the test in detecting pedophilia or hebephilia in complete nonadmitters is probably greater than or equal to 55% but is certainly less than 100%. Its specificity appears to be over 95%.

Child sexual abuse is one of the most destructive events for healthy child development. Following psychiatric classification systems, pedophilia must be distinguished from child sexual abuse. Approximately only one half of all child abusers fulfill the diagnostic criteria for pedophilia which is defined as a persistent or dominating sexual preference for prepubescent children characterized by persistent thoughts, fantasies, urges, sexual arousal or behavior. This article describes the diagnostic criteria and potential differential diagnoses as well as epidemiological and etiological findings. From an etiological point of view multifactorial mechanisms are currently considered to be responsible especially genetic factors, learning theoretical and neurobiological factors. Psychotherapeutic and pharmaceutical treatment options will be discussed. According to the current state of knowledge cognitive-behavioral psychotherapy is the method of choice in the treatment of pedophilia and has demonstrated positive treatment effects in meta-analyses regarding relapse prevention. Medicinal treatment of pedophilia is only indicated for severe forms of pedophilia. Important aspects of risk management in the treatment of pedophilia and aspects which must be considered in the forensic psychiatric assessment are presented.

The public stigma associated with pedophilia, the sexual attraction to prepubescent children, is tremendous. Previous research indicates that undifferentiated media coverage plays an essential role in perpetuating the public stigma by falsely equating pedophilia and child sexual abuse (CSA) and thus may stop persons suffering from a pedophilic disorder from seeking professional help. Until now, a comprehensive examination of positive as well as negative media effects on affected individuals is missing. Therefore, the present study explores if and how media coverage impacts the lives of help-seeking persons with pedophilia by conducting four qualitative focus group discussions with a clinical sample (N = 20) from the German Prevention Network "Kein Täter werden". Present results demonstrate that media coverage of pedophilia was perceived as mostly undifferentiated, even though participants observed an increase in fact-based reporting over the years. Moreover, it seems that media coverage has strong emotional and behavioral consequences for patients (e.g., negative reporting reduced self-esteem). In sum, our results highlight that differentiated media coverage could play a key role in supporting help-seeking persons with pedophilic disorder, while the impact of undifferentiated media coverage appears to be mostly negative. Therefore, our results point to the need to reframe pedophilia using differentiated media coverage to help affected persons receive treatment efficiently and thereby prevent CSA.

Paraphilias may occur with brain disease, but the nature of this relationship is unclear. The authors report 2 patients with late-life homosexual pedophilia. The first met criteria for frontotemporal dementia; the second had bilateral hippocampal sclerosis. Both were professional men with recent increases in sexual behavior. In both, 18-fluorodeoxyglucose positron emission tomography revealed prominent right temporal lobe hypometabolism. These cases and the literature suggest that bilateral anterior temporal disease affecting right more than left temporal lobe can increase sexual interest. A predisposition to pedophilia may be unmasked by hypersexuality from brain disease. These observations have potential implications for all neurologically based paraphilias.

Child sexual abuse is a serious, pervasive problem with clinical, social, moral, and legal implications. Between 100,000 and 500,000 children in the United States are thought to be sexually molested annually. Physicians in all specialties may detect sexual exploitation of youngsters and are mandated to report such cases. Failure to diagnose child molestation and pedophilia and to treat their cause can have serious, long-lasting consequences for innocent victims and continued distress for the perpetrator and for the professional who missed the diagnosis. A single child molester may commit hundreds of sexual acts on hundreds of children. The etiology of paraphilic syndromes is multifactorial. There are substantial differences among sexual abusers of children in their personalities and psychopathologies. Although available interventions are symptomatically palliative rather than curative, many pedophiles can benefit from appropriate treatment. Primary prevention may be the key to reducing the frequency of child sexual abuse.
