Physical treatment and rehabilitation play major roles in recovery and maintenance of the equine athlete, and many therapeutic measures are accessible by the veterinarian in general practice. An accurate diagnosis of the condition undergoing treatment is a requirement, and measurable parameters obtained at diagnosis allows for quantification of treatment outcomes. Therapeutic modalities accessible to the general practicing veterinarian are reviewed. Mechanisms of action, indications, and treatment protocols of thermal therapy, therapeutic ultrasound, extracorporeal shock wave, and laser are discussed. Manipulative therapies, including stretching and use of core strengthening exercises and equipment, are outlined.

The author presents a review of the experimental and clinical studies that have formed the basis of modern physiotherapy as a science, academic discipline, and clinical specialty. The analysis of the attributes of modern physical therapy has been undertaken with special reference to the definitions of the concepts, categories, classifications, therapeutic physical factors, and physical treatment methods that provide the basis of modern physical therapy. The directions of scientific and practical development of physiotherapy in the currently adopted system of organization of the medical care services in Russia are determined. The discussion presents arguments for the introduction of new concepts, terms, classifications, and treatment methods. The prerequisites for the formation of such notions and ideas, their composition and structure as well as the role of the therapeutic physical factors in the system of treatment and preventive measures are considered. The basic methodological approaches (both evidence-based and systematic) which are in line with modern physiotherapeutic research programs are substantiated. Special attention is given in the article to the currently adopted system of the training of physiotherapists accepting present-day ideas and practicing modern approaches in their practical work. The principal way to carry out this task under the present-day conditions is through education in a three-stage manner in accordance with the main categories of physical therapy. The current status and assessment of the use of the concepts of evidence-based and translational medicine in physiotherapy are discussed together with the prospects for its further systematic development and practical application.

This review assesses the role of physical modalities - chest physical therapy, breathing exercises, breathing training, and physical exercises - in the treatment of patients with pulmonary diseases. Controlled studies suggest that "routine" exercises yield a disappointing effort-benefit ratio, but that individually adapted programs may improve the well-being and functional ability of selected patients.

Physical therapy (physiotherapy, or PT) can be broadly defined as the restoration of movement and function and includes assessment, treatment, and rehabilitation. This review outlines the history, definition, and regulation of PT, followed by the core scientific principles of PT. Because musculoskeletal physiotherapy is the predominant subdiscipline in equine PT, encompassing poor performance, back pain syndromes, other musculoskeletal disorders, and some neuromuscular disorders, the sciences of functional biomechanics, neuromotor control, and the sensorimotor system in the spine, pelvis, and peripheral joints are reviewed. Equine PT also may involve PT assessment and treatment of riders.

This article on physiotherapy presents some current evidence stating the strengths and weaknesses of the physiotherapeutic procedures. In the area of physiotherapy empirical data obtained during decades were overtaken by evidence from current studies. The author points out the great problem of physiotherapy, namely the heterogeneity of the applied parameters. Knowledge of current evidence may be very important and helpful for the physicians, but the author proposes, from the practical point of view, that physiotherapeutical procedures based on exprience and used for many years should not be entirely neglected. Nowadays physiotherapy plays an important role in the treament of locomotor diseases but its use is increasing in other fields of medicine, as well.

Medical interventions focus on causation and disease processes, while rehabilitation, including most elements of physical therapy, is concerned with the consequences of disease. Its goals are to reduce symptoms, improve function and minimize disability. In rheumatology, the rehabilitation perspective mainly concerns patients with back pain, soft tissue and age-related musculoskeletal problems. In inflammatory diseases, anatomical and functional rehabilitation management includes joint protection, the maintenance of physical fitness and the prevention of impairment. This chapter will focus on essential therapeutic options and strategies in the rehabilitation of rheumatic disorders. It includes the evidence-based description of measurement instruments, joint mobilization, muscle techniques and physical modalities.

Musculoskeletal conditions are important contributors to United States healthcare spending and are certain to play an important role in the future as the population continues to age. Almost half of the population of the United States experiences a musculoskeletal condition annually. Meeting the needs of these individuals within a changing healthcare delivery and reimbursement environment prompted a recent Summit sponsored by the United States Bone and Joint Initiative (USBJI). The Summit dealt with a topic critical to the future of healthcare for clinicians, consumers, and payers alike: value. We do not operate within a value-based healthcare system. Our current delivery system continues to reward volumes, not value. Failure to focus on value has had devastating consequences. The challenge of shifting from a volume-based to a value-based system is central to the future of healthcare. Discussion and action will be critical for the physical therapy profession moving into the future.

Interest in manual therapy appears to continue to grow among physical therapy clinicians and educators throughout the world even though the underlying concepts and techniques have not been justified by a knowledge base. The purposes of this article are to critically assess the role of manual therapy within the physical therapy profession and to provide an introduction to the other articles in this special issue. Eisner's model of explicit, implicit, and null curricula is used as a framework for our analysis and our discussion of manual therapy. The explicit area of manual therapy includes discussions of the definition and the role of manual therapy, the scientific rationale for manual therapy, and manual therapy in education and a comparison of manual therapy evaluative frameworks. The implicit area deals with the role of clinical decision making and critical thinking in manual therapy in education and rehabilitation. In the null (unaddressed) area of manual therapy, we suggest directions for future development and research.
