Review
Epilepsy in children is particularly frequent. It may be severe because of cognitive function involvement. Some seizure types are specific of age. Epilepsy syndromes are useful for prognosis, treatment, and etiologic studies. Some syndromes result from various causes, others are difficult to distinguish at the onset. Recording sleep electroencephalogram and ictal polygraphy (including electromyogram) are major contributions in difficult cases.

OBJECTIVE
To design a diagnostic support system for sleep apnoea and hypopnoea syndrome (SAHS) using moving average based on knowledge, able to identify SAHS episodes from a respiratory polygraphy (RP) database.
METHODS
An analysis was made of data obtained from a public database, that included the RP signals, nasobucal airflow, thoracoabdominal movement, and pulse oximetry of 23 patients between 28 and 68 years with suspected SAHS, and with a body mass index (BMI) from 25.1 to 42.5.
RESULTS
The identification and classification of episodes of apnoea and hypopnoea was obtained.
CONCLUSIONS
The algorithm designed identified episodes of SAHS using polygraphy signals, which by implementating in a graphical interface allows visualisation of onset, duration, type, oxygen saturation, and pulse oximetry of each episode, and can be used as a support tool for the diagnosis of sleep disorders.

Post-conviction polygraph testing of sex offenders (typically referred to as PCSOT) is common in the US, increasingly applied in England and Wales, but hardly if ever used elsewhere in the world. This article provides an account of the nature and aims of PCSOT, research into its efficacy, and controversies associated with it.

Sleep Science (Sao Paulo, Brazil) 2018To establish the prevalence of positional (PP) OSA patients using self-administered home-based respiratory polygraphy (RP).
OBJECTIVE
To establish the prevalence of positional (PP) OSA patients using self-administered home-based respiratory polygraphy (RP).
MATERIALS AND METHODS
52 month retrospective study based on RP records.
RESULTS
200 PR records: 70.5% men 29.5% women. 76% were diagnosed with OSA and 54.6% with PP OSA. There were no significant differences in Epworth Sleepiness Scale, apnea hypopnea index and oxygen desaturation index. PP OSA patients were younger, had a lower BMI (30.3±0.9 vs. 35.3±1.2) (<0.0001), and the time they spent with oxygen saturation <90% (T<90) was lower (8.8 vs. 28.7±6.7, =0.0038). The PP OSA group spent 43% of total recording time in the supine position.
CONCLUSIONS
The prevalence of PP OSA patients studied with RP is similar to the one described by sleep laboratories. They have lower BMI, present mostly mild OSA with less desaturation, and are less likely to receive CPAP therapy.

OBJECTIVE
To overcome very few facilities available for polysomnography, a portable device of polygraphy was introduced into home monitoring for the assessment of obstructive sleep apnea syndrome (OSAS) in children.
METHODS
Forty-eight children (aged 2-11) presenting with snoring and sleep apnea were subjected to home monitoring. Sleeptester™ (Fukuda Lifetech, Japan) was used for this purpose, which was equipped with 5 channels for oronasal airflow, thoracoabdominal effort, snoring, body position, and oximetry (SpO2). Sensors were placed by guardians, and they were requested to attend their children as long as possible during a night. Results were analyzed manually by sleep technologists. Adenotonsillectomy was performed in all 48 children, and the same monitoring was utilized postoperatively.
RESULTS
The mean duration of monitoring was 460 ± 172 min. (Mean ± S.D.) in the preoperative test and 471 ± 126 min. in the postoperative test. The mean apnea-hypopnea index (AHI) was 20.6 ± 16.6 and 4.4 ± 2.1, respectively. There was a statistically significant decrease (p < 0.001). The lowest SpO2 value was 76.7 ± 17.1% preoperatively and 80.8 ± 14.6% postoperatively, demonstrating no significant difference (p = 0.16)
CONCLUSION
Attended home monitoring by guardians using a portable device can be useful in the perioperative assessment of pediatric OSAS.

Review
INTRODUCTION
Non epileptic paroxysmal events (NEPE) are clinical disorders of usually sudden appearance, brief and originated by a cerebral disfunction of diverse causes with a common character to be no epileptic.
DEVELOPMENT
The non epileptic paroxysmal events are more frequent than epileptic events. The 10% of the children have NEPE. The NEPE can to be confounded with epileptic fits. Careful and detailed history and wise valuation of the circumstances of occurrence and the characteristics and duration of the seizures are determined. The history include developmental milestones and search for possible causes of seizure disorders. A complete clinical examination and EEG are essentials in the differential diagnosis. More sophisticated techniques--polygraphy and video EEG monitoring--are of great value in selected and difficult cases.
CONCLUSIONS
The NEPE are categorized: anoxic/hypoxia paroxysmal disorders, psychogenic paroxysmal events, paroxysmal events occurring during sleep, paroxysmal events of movements and other episodic symptoms.

Sexual offenders are extremely reluctant to disclose their offending histories for a variety of psychosocial and legal reasons. The polygraph has shown promise as a intervention for eliciting admissions of past sexual offending behaviors. For 60 adult male sexual offender (35 inmates and 25 parolees), the number of victims and offenses were recorded from the Presentence Investigative Report, Sexual History Disclosure form, and 2 consecutive polygraph examination reports. Dramatic increases in the number of admitted victims and offenses were found for inmates, but not for parolees, across each source. However, there was a substantial decline in the number of victim and offense admissions by the second polygraph examination for both groups, even though 80% of the examination results reveled deception about sexual offending behaviors. Standardized use of sanctions and privileges for deceptive and nondeceptive polygraph results, respectively, are proposed as a way of eliciting full disclosure of offending histories for these offenders.
