Polymyalgia rheumatica is a rheumatic disease which mainly affects the elderly, and is seldom diagnosed in patients <50 years of age. The prevalence of polymyalgia rheumatica is approximately 16.8 to 53.7 per 100,000 of the population >50 years of age. Patients may present with spiking fever, malaise, fatigue, weight loss and other features suggesting inflammation, which in each case requires differential diagnosis from malignancies. Neoplastic disease in turn can manifest itself in symptoms resembling those of polymyalgia, which are named "polymyalgia-like syndrome" and are in fact paraneoplastic syndromes presenting as polymyalgia rheumatica. These observations suggest that a careful clinical evaluation and a long term follow-up are necessary for a correct diagnosis of polymyalgia rheumatica.

A case is reported of rheumatic polymyalgia rarely amenable to correct diagnosis. The response was achieved after glucocorticoid treatment. The diagnosis was based on the following criteria: age over 50, muscular pain (in two or more regions of the neck, shoulder and pelvic girdle, ESR above 35 mm/h, duration of the symptoms at least 2 months, limited movements in the cervical spine, shoulder and hip joints, weakness, subnormal appetite, fever, anemia, loss of weight.

Despite being the most common inflammatory rheumatic disease in the elderly (Partington et al., Ann Rheum Dis 77(12):1750-175, 2018), few studies to date have examined the patient experience of polymyalgia rheumatica (PMR). Our study explored patient perspectives in PMR by means of a survey and semi-structured group interviews. The results from this study highlight key aspects of the patient experience in PMR, including delays to diagnosis, complex attitudes toward glucocorticoid therapy, and a desire for alternate treatment strategies. Future trials should look to include physical function as a measured outcome. Finally, our results call attention to the chronicity of PMR symptoms, challenging the paradigm of PMR as a self-limiting condition.

The diagnostic and therapeutic strategies for polymyalgia rheumatica (PMR) have changed substantially in recent years. Rather than a single disease entity, PMR has emerged as a syndrome produced by a variety of conditions. The diagnostic criteria that have been used for several decades are inadequate. These facts support a new and broader pathological concept, polymyalgic syndrome, and a standardized diagnostic and therapeutic approach designed to rule out diseases with misleading presentations and to identify the limited number of patients with polymyalgic syndrome who have PMR. Criteria for both polymyalgic syndrome and PMR were developed recently but remain to be validated. These criteria are discussed, as well as the suggested diagnostic approach and treatment strategy. In contrast, studies on pathophysiological models, inflammatory mechanisms, and genetic factors are not considered herein, as they were conducted in heterogeneous populations of patients who did not meet the new criteria. Current data indicate that polymyalgic syndrome is a mode of onset of inflammatory joint disease in individuals older than 50 years of age and not (in most cases) a disease entity.
