Filling the bladder with 400--500 ml of saline displaces the presenting part and relieves pressure on the prolapsed umbilical cord. With this method there were no perinatal deaths in 24 patients with prolpsed cord, although the average interval between diagnosis and delivery was 65,0 minutes. This method has several advantages over other first-aid measures to relieve pressure on the prolapsed cord.

Hernia of umbilical cord is a well-known entity which presents with herniation of small bowel into the proximal part of umbilical cord. It has very good prognosis after surgical repair. Occasionally, it can have distinct presentations and varied malformations at the umbilicus which have bearing on the course of treatment and final outcome. Herein, we describe various presentations and malformations associated with hernia of umbilical cord. Embryological extrapolation is attempted for the malformations at umbilicus.

With the improvement of prenatal diagnoses of foetuses, the prevalence of stillbirth due to foetal anomaly after mid-gestation decreased, whereas that of stillbirth associated with umbilical cord factors tended to increase. Prenatal detection of umbilical cord abnormalities and appropriate management during the antenatal period and delivery based on the ultrasound diagnosis will improve the perinatal morbidity and mortality rates. In the present review, the strategy to reduce the incidence of foetal compromise due to umbilical cord problems is discussed considering the current knowledge regarding the physiological and pathological aspects of umbilical cord abnormalities.

Prolapse of the umbilical cord is a rare obstetric emergency that in the viable fetus necessitates an expeditious delivery. A case of a periviable pregnancy complicated by preterm premature rupture of membranes and overt umbilical cord prolapse was prolonged 2 weeks with expectant management is described. An extensive review of the literature regarding the etiology, risk factors, and management options for umbilical cord prolapse in both viable and previable pregnancies accompanies this report.

Umbilical cord prolapse occurs when a loop of cord is present below the presenting part when the amniotic membranes are ruptured. The incidence is 0.2% of total births. The case presented here is unusual because the definition of cord prolapse cannot be applied to it and the lady did not complain of any history of urinary incontinence or hematuria. The presentation of umbilical cord through maternal urethra led to the speculation of an opening between the anterior uterine wall and the bladder. Fetal demise was diagnosed by abdominal ultrasound. At laprotomy, a communication was found between the bladder and the uterus thus letting the cord traverse through the bladder.

An emergent cesarean delivery is performed to immediately intervene to improve maternal or fetal outcome for such indications as fetal distress, prolapsed cord, maternal hemorrhage from previa or trauma, uterine rupture, and complete placental abruption. It is paramount to reduce morbidity and mortality by preparing health care providers for special precautions.
