This paper addresses the issues of the labeling and selective discrimination of homosexuals. Psychiatry is encouraged to adhere to the medical model and treat the homosexual patient who is in conflict and motivated by inner turmoil and distress rather than assuming the job of society's regulator and the judge of acceptable behavior. The author discusses the concept of gender identity and presents evidence for the idea that homosexual behavior has its etiology in early child development. The cultural stigma of gender dysphorias is also discussed in detail as playing a sigificant role in the eventual character formation of an adult homosexual.

The desire to identify with and assume the role of the opposite gender has been present with us since antiquity. Unfortunately, the syndrome of transsexualism as defined by the DSM-III encompasses many individuals who are seeking a gender change perhaps allowing reassignment to take place when not indicated. This paper reviews the major classifications applied to the condition of transsexualism and a new model of classification for individuals with Gender Dysphoria is outlined. It will be shown that by subdividing the condition into those with the hypothesized syndrome (primary (true) transsexualism, or the symptom, secondary transsexualism) a better understanding of gender dysphoria is achieved. By applying this new model of classification, a management/treatment protocol is outlined which is helpful in dealing with the gender dysphoric patient.

Psychosexual health of 308 children, aged 2-11 years, as well as that of their families, was studied. Deviations in psychosexual development were found in 34.6% of the children examined. The following types were detected: difficulties in formation of gender-determined behavior features--64.4%, precocious psychosexual development--13.7%, delayed psychosexual development--12.3%, obsessive masturbation--9.6%. Risk factors for deviant psychosexual development were found.

This paper uses sociological theory to develop an ideal-typical model of homosexual identity formation. The four-stage model outlined here represents a synthesis and an elaboration on previous research and theorizing on homosexual identity development. The model describes how committed homosexuals, lesbians and gay males who see themselves as homosexual and adopt corresponding lifestyles, recall having acquired their homosexual identities. Often repeated themes in the life histories of gay males and lesbians, clustered according to life stages, provide the content and characteristics of each stage.

Numerous contemporary analyses have challenged traditional models of female psychosexual development, identifying their norms as implicitly androcentric. Femaleness has been regarded as derivative or deviant, as less and "other" than maleness. Alternative, woman-centred, models emphasise three major aspects of female psychosexual development: affiliation (women and others), maternity (women as mothers), and alterity (women and otherness). For some theorists the affiliative/maternal emphasis constructs a model of healthy and desirable femaleness. For others it represents not only the outcome of millennia of female subordination but also provides a justification for perpetuation of that subordination. Whatever the resolution of these dilemmas, a woman-centred perspective of female psychosexual development is relevant to contemporary psychiatric thinking. This review will present such a perspective and raise some of those dilemmas.
