Sarcina ventriculi is a rare gram-positive, anaerobic bacteria, associated with delayed gastric emptying. We report a case of a 45-year-old lady, who presented with features of gastric outlet obstruction, and coinfection of S. ventriculi and Candida was detected on examining gastric brushings and biopsy. S. ventriculi is identified by its peculiar configuration in the form of tetrads and octets. Coexistence of S. ventriculi with other organisms is highly unusual.

Acquired antral pyloric hypertrophy is one of the most common causes of pyloric obstruction in the small (10 kg) mature or old dog. Clinical signs include chronic intermittent vomiting after meals and occasional weight loss or abdominal distension. Definitive diagnosis is made by exploratory laparotomy and histologic examination of excised tissues. Most dogs with acquired antral pyloric hypertrophy become clinically normal after surgical correction of the gastric outlet obstruction.

Gastric emptying of liquids and solids was evaluated in ten dogs with gastric fistula before and after either truncal vagotomy, antrectomy, or pyloroplasty. In subsequent operations, the vagotomy group underwent either antrectomy or pyloroplasty, and the antrectomy and pyloroplasty groups were vagotomized. Vagotomy significantly inhibited the emptying of both liquids and solids (p less than 0.001) when the pyloric antrum was intact. When the antropyloric mechanism was removed by antrectomy, the effect of vagotomy was to accelerate the emptying of liquids (p less than 0.05) but to impair the emptying of solids (p less than 0.001). When the pylorus was destroyed by pyloroplasty, vagotomy markedly accelerated the emptying of liquids but had little effect on the emptying of solids. Neither antrectomy nor pyloroplasty had any significant effect on the emptying of solids. Pyloroplasty caused minor acceleration of emptying of liquids while antrectomy had no effect.

A 34-year-old man suffered intermittent abdominal pain for 1 month, especially after drinking or eating. Enhancement CT was performed to determine the cause, which detected a soft tissue mass in the gastric antrum, suggestive of possible malignancy. FDG PET/CT scan was undertaken for staging, which showed increased metabolism in the known gastric mass, also suggestive of malignancy. However, this lesion was confirmed as ectopic pancreatitis pathologically.
