BACKGROUND
COVID-19 created an unexpected situation for dental patients. However, the impact of the lockdown on orthodontic patients is still unknown. Hence, this study was conducted to analyse the number and type of emergencies, distress and fear of getting infected among brackets and aligners orthodontic patients during the COVID-19 lockdown.
MATERIAL AND METHODS
A total of 324 questionnaires about emergency appointments during the lockdown and Kessler Psychological Distress Scale-K10 were distributed between March and December 2022 among patients who were on active orthodontic treatment during lockdown at the Master of Orthodontics at the University Alfonso X el Sabio during February and December 2022. Chi-square was used to compare groups.
RESULTS
Emergency appointment was needed by 37.78% of patients during lockdown: 73.5% among those with aligners and 45.9% with brackets (<.01). Loose bracket was the most prevalent emergency for bracket patients and lack of aligners among invisible orthodontic patients. Of those, 36.95% in need of orthodontic emergency appointments didn´t attend the clinic for fear of being infected; 21% of patients had high or very high anxiety levels.
CONCLUSIONS
Orthodontic patients had moderate levels of anxiety and fear of being infected during lockdown. Of these, 31.78% needed an emergency appointment. Orthodontics, COVID-19, dental healthcare, dentistry, emergency.

BACKGROUND
Universal adhesive systems used for restorative clinical procedures are like orthodontics and may be a viable option. This study evaluated the effectiveness of universal adhesive systems in enhancing the durability of the shear bond strength (SBS) and adhesive remnant index (ARI) of orthodontic brackets to enamel.
MATERIAL AND METHODS
100 bovine incisors were divided into five groups (n=20), according to the applied adhesive systems: Primer Transbond XT; Ambar; Ambar Universal; Single Bond Universal; Adper Single Bond 2. Bracket from each tooth were submitted to SBS test after 24 hours, and 12 months later. The amount of remaining adhesive was evaluated through ARI.
RESULTS
After 24 hours, there was no difference in BS between the control and the other groups (p>0.05). However, there were difference between TOTALETCHING1 group and the Ambar Universal (=0.015) and Single Bond Universal groups (=0.011). After 12 months, Primer Transbond XT, Ambar, Ambar Universal and Adper Single Bond 2 showed no differences in the SBS (>0.05). Nonetheless, Single Bond Universal presented superior result when compared to Primer Transbond XT (=0.046) and Ambar (=0.011) groups. The SBS of all groups reduced significantly after 12 months (<0.05). There was no difference between ARI scores in each individually assessed group (>0.05), for both periods. Following 24 hours, a difference was observed between the groups (=0.043), fact that didn't occur after 12 months (=0.109).
CONCLUSIONS
Adhesive systems, such as Ambar Universal and Single Bond Universal are efficient in bonding orthodontic brackets to enamel when associated with Transbond XT adhesive paste. Bond strength; Primer Transbond XT, orthodontic brackets, adhesive systems.

Background Monocortical mini-screw-type temporary anchorage devices (TADs), or mini-screws, have significantly impacted orthodontic treatment strategies, especially in severe crowding and protrusion cases. These devices offer flexibility in placement sites, but the chosen location can considerably influence tooth displacement patterns. Key factors include the 'line of force' and the biomechanical properties of orthodontic tools. By analyzing tension distribution and three-dimensional displacements, the finite element method (FEM) provides a thorough means to comprehend these patterns. The Curve of Spee (COS) is a crucial factor potentially affecting displacement. Objective This study aimed to leverage finite element analysis (FEA) to understand the impact of varying mini-implant heights (10 mm, 13 mm, and 16 mm) on the displacements of different tooth types under a consistent force of 150 gm and compare these displacements both in the presence and absence of the COS. Materials and methods A CAD model of the jaw and teeth was developed using CT scan data and a Rexcan III 3D White Light Scanner. This model was meshed in Altair HyperMesh using tetrahedral elements, resulting in a Finite Element Model. The model incorporated various components, including teeth, the periodontal ligament (PDL), alveolar bone, brackets, a titanium mini-screw, and an archwire measuring 0.019 x 0.025 inches. Unique material properties were assigned to the PDL, and the assembly accurately replicated the clinical alignment of the archwire and brackets. Subsequently, stress and strain analyses were conducted on the model using the FEM. Results The displacement patterns of various teeth at implant heights of 10 mm, 13 mm, and 16 mm under a 150-gm force were analyzed in relation to the COS. Notably, for the central incisor, the COS significantly affected displacements in the Y and Z directions. Similarly, the Lateral Incisor and Canine exhibited marked changes in the Z direction with the presence of the COS. The Second Premolar's apex displacement showed significant variation due to the COS, while the First Molar displayed notable changes in the X direction. Generally, the presence of the COS either maintained or slightly increased Z-directional displacements across teeth, particularly at the apices. Conclusion The presence of COS significantly influences tooth displacement patterns when using mini-screws at different implant heights. Central incisors, lateral incisors, and canines are particularly sensitive to changes in the Z direction with the COS. The biomechanical analysis emphasizes the importance of considering COS in treatment planning for optimal results with mini-implants in orthodontics.

Is significant mandibular advancement possible after the peak of puberty? Dento-osseous palatal expansion and the STM4 technique (Skeletal Therapy Manni Telescopic Herbst 4 miniscrews): A case report.
INTRODUCTION
Treatment of skeletal class II growing patients often requires the use of functional appliances, aimed at promoting mandibular advancement. Among these, Herbst appliance is recommended for its effectiveness, efficiency, and reduced need for compliance. Despite its skeletal favourable effects, well-known dental compensations can occur, especially when the appliance is not used close to the pubertal peak: upper incisors retroclination, lower incisors proclination, upper molars distalization and lower molars mesialization could reduce the overjet needed for a proper mandibular advancement. To counteract these unfavourable effects skeletal anchorage could be crucial.
AIM
The aim of this case report is to describe and evaluate the effects of using a skeletally anchored Herbst appliance in an 18-year-old (CVM5) male patient with skeletal Class II malocclusion and a convex profile.
TREATMENT PROTOCOL
The treatment started with a tooth-bone-borne palatal expansion, then the upper arch was bonded with pre-adjusted ceramic brackets. After 2months, a Manni Telescopic Herbst (MTH) supported by 4 miniscrews (two in the maxilla and two in the mandible) was applied. To avoid anchorage loss, TADs were connected with elastic chains to the arches. Nine months later, the Herbst was removed, the lower teeth were bonded and the patient wore class 2 elastics to stabilise the occlusion.
RESULTS AND CONCLUSIONS
After 24months the treatment goal was achieved with a considerable improvement of the profile and a clinically significant mandibular advancement (Pogonion moved forward 7mm). A one-year follow-up lateral X-rays showed a good stability of the result.

Throughout treatment with fixed orthodontic appliances, effective plaque control is crucial to maintaining dental health. This in-vitro study evaluated the cleaning performance of eleven different brush heads of seven electric toothbrushes (oscillating-rotating and sonic motions) and varying brushing forces around orthodontic brackets. Six Mini Diamond Twin brackets were placed on black-stained front teeth. Teeth were coated with white titanium oxide and brushed in a machine six times for one minute with two different brushing forces (1 N and 1.5 N). Eleven different brush heads were evaluated (either oscillating-rotating or sonic movements). The teeth were scanned and planimetrically evaluated after brushing. Three detailed plaque areas (DPAs) were created: proximal (< 1 mm to bracket), mid-tier (1-2 mm to bracket), and distant (> 2 mm to bracket). The proportion of contaminated proximal, mid-tier, and distant surfaces (white regions) in relation to the respective DPA was calculated. Independent of brushing forces, places with a higher distance (> 2 mm) to the orthodontic bracket had the least amount of residual contamination, followed by areas with a minor (1-2 mm) and proximal distance (< 1 mm). In all of the brushes tested and for both estimated brushing forces, the region with the highest residual contamination was the proximal area. The brush heads of the Paro Sonic toothbrush left the least amount of residual contamination. The cleaning performance of electric toothbrushes around brackets on upper incisors varied across the brushes examined. The proximal area has the most residual contamination. Furthermore, 9 out of 11 toothbrushes cleaned more successfully with 1.5 N than with 1 N brushing force.

BACKGROUND
The objective is to compare the impact of clear aligner treatment (CAT) versus conventional fixed appliance treatment (FAT) on oral health-related quality of life (OHRQoL) among adults at five-time points: pretreatment (T0), 1 week (T1), 1 month after (T2), and 6 months after (T3) treatment initiation, and in the long-term follow-up (T4).
MATERIALS AND METHODS
Search terms were based on Medical Subject Headings (MeSH) and non-MeSH. Potentially eligible studies compared OHRQoL in clear aligner (CA) and fixed appliance (FA) patients. In February 2023, Scopus, Web of Science, Cochrane, and PubMed were searched for published studies. Nine out of 94 shortlisted papers were eligible for a systematic review. Of these nine papers, five studies were considered for a meta-analysis.
RESULTS
At T0, CA and FA patients had similar oral health impact profile (OHIP)-14 questionnaire scores with a standard mean difference (SMD) of 0.105 (confidence interval [CI]: -1.029-1.48). The SMD of the OHRQoL related to T1, T2, and T3 was -3.119 (CI: -0.145, 0.355), -1.527 (CI: -5.597, -0.64), and - 2.331 (CI: -1.906, -1.148). T4 showed no difference between groups (SMD = 0.007, CI: CI: -4.286, -0.376). Regarding the OHIP-14 domains, functional limitations remained consistent in both groups across all time intervals. Psychological discomfort exhibited a notable difference only at T2. Throughout the treatment, CAT showed significantly lower levels of physical, psychological, and social disability, as well as handicap, though these differences did not persist beyond T4. Notably, physical pain was the sole domain that remained elevated in the FAT group up to T4.
CONCLUSION
During the 1 day of the orthodontic treatment, both the CA and FA groups had comparable OHRQoL statuses. However, as time passed, the CA group notably improved their OHRQoL compared to the FA group. Interestingly, after a year or the completion of treatment, both groups eventually reached similar OHRQoL levels. Nevertheless, it is worth noting that FA patients continued to experience more physical pain even a year later.

OBJECTIVES
This study evaluated the effects of adding silver nanoparticles on the shear bond strength, microhardness, and surface roughness of orthodontic adhesives.
MATERIAL AND METHODS
Fifty upper premolars were randomly allocated to five groups (n = 10). Orthodontic brackets were bonded with silver nanoparticle (AgNP)-modified adhesives (1 %, 0.5 %, 0.1 %, 0.05 %), and conventional adhesive was used as a control. The shear bond strength was recorded using a universal testing machine, and the adhesive remnant index was evaluated using a stereomicroscope. Ten discs of each adhesive were subjected to the microhardness and surface roughness tests. The Vickers microhardness values were measured under a constant load of 100 g for 30 s using a microhardness tester. The samples were analyzed using a surface profilometer, and the arithmetic average roughness was used as the measurement parameter. Data were analyzed using one-way analysis of variance and chi-square tests. A significance level of 5 % was considered significant.
RESULTS
AgNP concentration > 0.1 % significantly reduced the shear bond strength (p < 0.05). At higher AgNP concentration, the bonding failure pattern occurred mainly at the bracket-resin interface. The Vickers microhardness increased with increasing concentration, and significant differences were observed between the group with 1 % AgNP and the other groups (p < 0.05). The average roughness values were similar between the groups with AgNP concentrations > 0.1 % (p > 0.05).
CONCLUSION
The incorporation of AgNP into an orthodontic adhesive has the potential to decrease the shear bond strength while increasing the microhardness and surface roughness.

Minimally invasive periodontic (perio) surgical procedures, piezocision, and micro-osteoperforation are useful techniques for accelerating tooth movement. These techniques also offer advantages in the orthodontic (ortho) and aesthetic domains. This study aimed to evaluate and compare the rates of lower anterior decrowding with piezocision and micro-osteoperforation.
METHODS
This clinical study included 24 patients requiring fixed orthodontic treatments. Two periodontic techniques (piezocision (PZ) and micro-osteoperforation (MOP)) were considered for the orthodontic treatments. Each patient was randomly allocated to either the piezocision (PZ) group or the micro-osteoperforation (MOP) group. The piezocision group received five radiographically guided incisions on the labial surface of the alveolar bone, whereas the micro-osteoperforation group received one to three MOPs each using a mini-implant drill between the six lower anterior teeth, and later, an initial arch wire was ligated to each bracket. Little's irregularity index (LII) was calculated using a digital vernier caliper on study models every four weeks until decrowding was achieved. The difference in the rates of lower anterior crowding between the piezocision and micro-osteoperforation groups was analyzed to determine the statistical significance.
RESULTS
The rates of irregularity index change during decrowding were 4.38 ± 0.61 in the piezocision group and 3.82 ± 0.47 in the micro-osteoperforation group. Piezocision was found to be 1.2 times faster than micro-osteoperforation in terms of the rate of decrowding.
CONCLUSION
The advanced perio-ortho combination technique was advantageous in accelerated decrowding. In comparison to MOP, there was an increase in the rate of decrowding with PZ. Decrowding can be completed quickly with PZ, and it can thus be used to treat crowding effectively in a limited time frame.

En-bloc retraction is a common procedure in orthodontic therapy. The application of palatal root torque moments is required to control incisor inclination during retraction, yet studies comparing forces and moments with respect to different mechanics are lacking. This study aimed to investigate the forces and moments during orthodontic en-bloc retraction using a robotic biomechanical simulation system, comparing two distinct approaches: (I) compound technique [stainless steel (SS) combined with nickel-titanium (NiTi)] using industrially pretorqued retraction-torque-archwires (RTA) in combination with NiTi closed coil springs; (II) conventional sliding mechanics using SS archwires with manually applied anterior twist bends in combination with elastic chains. Two dimensions (0.017" × 0.025" and 0.018" × 0.025") and ten archwires per group were investigated using 0.022" slot self-ligating brackets. Kruskal-Wallis tests with a significance level of α = 0.05 were conducted. The biomechanical simulation showed that en-bloc retraction was characterized by a series of tipping and uprighting movements, differing significantly regarding the examined mechanics. Collateral forces and moments occurred in all groups. Notably, RTA exhibited fewer extrusive forces. The most bodily movement was achieved with the compound technique and the 0.018" × 0.025" RTA. Sliding mechanics exhibited maximum palatal root torque moments of more than 20 Nmm, exceeding recommended values.
