Unusual Metastatic Sites and Radioiodine Uptake in Patients of Differentiated Thyroid Carcinoma with Atypical Clinical Presentations: Utilization of I-Whole-Body Scintigraphy with Regional SPECT/CT.
Differentiated thyroid carcinoma (DTC) usually is slow growing and carries a good prognosis. It most commonly tends to spread locally to regional lymph nodes in 20 to 60% of patients. The presence of distant metastasis impacts overall survival and prognosis. The lungs, bones, and the brain are typically involved in distant sites with less common metastatic sites that include the liver, kidney, skeletal muscle, adrenal glands, bladder, and skin. These unusual sites are rare and pose a diagnostic challenge and impact clinical decision-making to a great extent. The radioiodine I whole-body scintigraphy with single-photon emission computed tomography/computed tomography can provide a thorough investigation of unusual sites of uptake leading to diagnosis of these metastases. We present a case series of DTC showing unusual sites of metastasis and/or radioiodine uptake in urinary bladder, in the third metacarpal bone of left hand and lastly in the forearm at postoperative hypertrophic scar area.

BACKGROUND
This study evaluates the change in an MRI of the proximal metacarpal region in a group of sport horses that returned to work. This retrospective analysis evaluated 18 limbs represented by 17 horses.
RESULTS
The hyperintense signal within the dorsal collagenous part of the proximal suspensory ligament (PSL) on T1W/T2*W GRE sequences decreased or stayed the same in the majority of cases. The hyperintense STIR signal within the dorsal collagenous part of the PSL resolved in the majority of the patients, and the third metacarpal bone (McIII) hyperintense STIR signal resolved in all patients. The dorsal margin irregularity of the PSL stayed the same, and McIII sclerosis and resorption of the palmar margin of McIII stayed the same in the majority of cases. McIII hyperintense STIR signal resolution carries a broad time range, with a mean of 94 days and a range of 47-202 days.
CONCLUSIONS
Complete normalization of the dorsal collagenous part of the PSL does not appear necessary for a return to soundness, but a resolution of the McIII hyperintense STIR signal is expected for horses returning to soundness. A rescan period of 120 days for the proximal metacarpal region is suggested. In addition, there was no significant change in the size of the PSL between the initial and final MRI.

The emergence of 7T clinical MRI technology has sparked our interest in its ability to discern the complex structures of the hand. Our primary objective was to assess the sensory and motor nerve structures of the hand, specifically nerves and Pacinian corpuscles, with the dual purpose of aiding diagnostic endeavors and supporting reconstructive surgical procedures. Ethical approval was obtained to carry out 7T MRI scans on a cohort of volunteers. Four volunteers assumed a prone position, with their hands ( = 8) positioned in a "superman" posture. To immobilize and maintain the hand in a strictly horizontal position, it was affixed to a plastic plate. Passive B0 shimming was implemented. Once high-resolution 3D images had been acquired using a multi-transmit head coil, advanced post-processing techniques were used to meticulously delineate the nerve fiber networks and mechanoreceptors. Across all participants, digital nerves were consistently located on the phalanges area, on average, between 2.5 and 3.5 mm beneath the skin, except within flexion folds where the nerve was approximately 1.8 mm from the surface. On the phalanges area, the mean distance from digital nerves to joints was approximately 1.5 mm. The nerves of the fingers were closer to the bone than to the surface of the skin. Furthermore, Pacinian corpuscles exhibited a notable clustering primarily within the metacarpal zone, situated on the palmar aspect. Our study yielded promising results, successfully reconstructing and meticulously describing the anatomy of nerve fibers spanning from the carpus to the digital nerve division, alongside the identification of Pacinian corpuscles, in four healthy volunteers (eight hands).

A New Biosynthetic 6-Phytase Added at 500 Phytase Unit/kg Diet Improves Growth Performance, Bone Mineralization, and Nutrient Digestibility and Retention in Weaned Piglets and Growing-Finishing Pigs.
Two experiments were performed to evaluate the effect of a biosynthetic 6-phytase added at 500 phytase unit (FTU)/kg diet on growth performance, bone mineralization, and nutrient digestibility and retention in weaned piglets and growing-finishing pigs. Experiments were performed on 90 weaned male and female piglets with an average initial body weight (BW) at 7.7 ± 0.73 kg, 26 days of age) and 300 male and female growing pigs (initial BW: 21.0 ± 3.44 kg) for 43 and 98 days in experiments 1 and 2, respectively. In each experiment, the animals were assigned to one of three treatments according to a randomized complete block design. The treatments consisted of a positive-control (PC) diet formulated to meet nutrient requirements; a negative-control (NC) diet reduced similarly in calcium (Ca) and digestible P by 0.15 and 0.12% points in phases 1 and 2, respectively, in piglets and by 0.14, 0.11, and 0.10% points, respectively, in phases 1, 2, and 3 in growing-finishing pigs, compared with PC diet; and a NC diet supplemented with the new 6-phytase at 500 FTU/kg diet (PHY). The dietary P and Ca depletion reduced ( < 0.05) the final BW (-11.9%; -7.8%,), average daily gain (ADG, -17.8%; -10.1%), average daily feed intake (ADFI, -9.9%; -6.0%), gain-to-feed (G:F) ratio (-8.9%; -4.6%), and apparent total tract digestibility (ATTD) of P (-7.7% points; -6.7% points) in nursery piglets and growing pigs, respectively. It also decreased ( < 0.001) P and Ca retention by 6.1 and 9.4% points, respectively, in nursery pigs and ash, P, and Ca contents in metacarpal bones by 18.4, 18.4, and 16.8%, respectively, in growing pigs. Compared to animals fed the NC diet, phytase supplementation improved ( < 0.001) the final BW (+7.7%; +11.3%), ADG (+12.5%; +15.0%), G:F ratio (+8.4%; +5.8%), ATTD of Ca (+10.8% points; +7.2% points), and ATTD of P (+18.7% points; +16.6% points) in weaned piglets and growing pigs, respectively. In addition, phytase also increased ( < 0.001) P and Ca retention by 6.1 and 9.4% points, respectively, in nursery pigs and ash, P, and Ca contents in metacarpal bones by 17.7, 15.0, and 15.2%, respectively, in growing pigs. The final BW, ADG, G:F ratio, and bone traits in animals fed the NC diet supplemented with phytase were comparable to animals fed the PC diet. This finding indicates the ability of this novel biosynthetic phytase to restore performance and bone mineralization by improving the availability of P and Ca in piglets and growing pigs fed P- and Ca-deficient diets.

Periosteal sleeve fractures, or avulsions of cartilage and/or periosteum with or without an osseous fragment in skeletally immature individuals, are notoriously easy to miss and a high index of suspicion is necessary for accurate diagnosis and treatment. While periosteal sleeve avulsion fractures are classically reported in the patella, they have also been reported in the shoulder, clavicle, and elsewhere in the knee. However, no published reports exist for a periosteal sleeve avulsion fracture in the hand. This case details the first reported instance of such an injury involving a thumb metacarpal in a 3-year-old boy, treated with open reduction and percutaneous pinning of the thumb metacarpal.

OBJECTIVE
To describe the process whereby the screening of racing Thoroughbreds with accelerometer-based inertial measurement unit (IMU) sensors followed by clinical evaluation and advanced imaging identified potentially catastrophic musculoskeletal injuries in 3 horses.
ANIMALS
3 Thoroughbred racehorses.
CLINICAL PRESENTATION
All cases demonstrated an abnormal stride pattern either during racing (cases 1 and 2) or while breezing (case 3) and were identified as being at very high risk of catastrophic musculoskeletal injury by an algorithm derived from IMU sensor files from > 20,000 horses' race starts. Veterinary examination and 18F-sodium fluoride (18F-NaF) positron emission tomography were performed within 10 days of the respective race or breeze in each of the cases.
RESULTS
The intensity and location of the 18F-NaF uptake in the condyles of the third metacarpal bone in cases 1 and 2 identified them as at potential increased risk of condylar fracture. The pattern and intensity of the 18F-NaF uptake in case 3 indicated that the third carpal bone was likely responsible for the horse's lameness, with an impending slab fracture subsequently identified on radiographs. Following periods of convalescence, cases 1 and 2 returned to racing and were identified by the sensor system as no longer being at high risk of catastrophic musculoskeletal injury. Case 3 returned to training but has yet to return to racing.
CLINICAL RELEVANCE
When worn by Thoroughbreds while racing or breezing, these IMU sensors can identify horses at high risk of catastrophic musculoskeletal injury, allowing for veterinary intervention and the potential avoidance of such injuries.

OBJECTIVE
To describe a rare congenital deformity of the phalanges and the surgical details and outcome in a dog with ectrodactyly combined with polydactyly.
STUDY DESIGN
Single case report.
ANIMAL
A 3.5-month-old male intact mixed breed dog with forelimb lameness and paw malformations.
METHODS
Surgery was performed on a dog with a congenital limb deformity consisting of resection of the extra bone and soft tissue structure to prevent further subluxation of the remaining metacarpals. Stabilisation consisted of a cortical screw in compression and a K wire across the proximal metacarpals.
RESULTS
Postoperative radiographs showed adequate implant positioning and good reduction of the proximal metacarpal row. At six weeks, the dog showed improvement in limb function and weight bearing. Major complications occurred at twelve weeks, and revision surgery with implant removal was required. At six months, the dog showed near normal range of motion and no lameness.
CONCLUSIONS AND CLINICAL RELEVANCE
The decision to perform surgery on a dog with limb deformity resulted in an almost physiological gait, and the dog showed no abnormalities in daily life. This report adds to the literature on congenital limb deformities by describing the combination of ectrodactyly and polydactylism in a canine species, including the surgical approach and outcome. However, the optimal management of this heterogeneous condition is currently unclear.

Spatially offset Raman spectroscopy (SORS) is valuable for noninvasive bone assessment but requires a clearer understanding of how offset distances influence detection depth. To address this, our study devised a forward-adjoint Monte Carlo multi-layer (MCML) model to simulate photon paths in SORS, aiming to determine optimal offsets for various tissue types. We examined photon migration at offsets between 0 and 15 mm against layered phantoms of differing thicknesses and compositions to optimize the signal-to-noise ratio for bone layers. The findings highlight that optimal offsets are contingent on tissue characteristics: a metacarpal beneath 2.5 mm of tissue had an ideal offset of 6.7 mm, while a tibia with 5 mm of soft tissue required 10-11 mm. This precise calibration of SORS via MCML modeling promises substantial improvements in bone health diagnostics and potential for expansive medical applications.

BACKGROUND
Fifth metacarpal neck fractures (FMCNFs) are common among paediatric patients. Complications include reduced metacarpophalangeal (MCP) range of motion and grip strength, which impede the hand's functional abilities. Various management options are available, but indications are not standardised. This systematic review aims to assimilate all available evidence on the management of paediatric FMCNFs to determine appropriate treatment pathways.
METHODS
PubMed (Medline), EMBASE, Scopus and Google Scholar were used to identify evidence pertaining to the management of these fractures.
RESULTS
Ten studies were identified, involving 237 patients with a mean age of 14.4 years (Range 9-17). Ninety percent of patients were male. Sixty-one (26%) patients, with an average fracture angulation of 27° (Range 16°-33°) and no rotational deformities, were managed with immobilisation alone. These patients returned to normal metacarpophalangeal range of motion and grip strength. Fifty-four (23%) patients, with an average fracture angulation of 42.7° (Range 33°-54°), were managed with fracture reduction and immobilisation. This technique did not yield sustained reduction of fractures with significant angulation or rotation after intervention. One hundred twenty-two (51%) patients, with an average fracture angulation of 48.3° (Range 30°-58°) and including cases of malrotation, were managed with fracture reduction and surgical fixation. These patients experienced good functional outcomes.
CONCLUSIONS
This review suggests paediatric FMCNFs can be safely managed with immobilisation alone when there is an absence of rotational deformity and an angulation of < 30°. In the case of a higher fracture angulation or rotational deformity, fracture reduction and surgical fixation is an appropriate method of management.

A comparative analysis of 26 petrous bones and epiphyses of metacarpals from the Second World War era revealed no significant differences in DNA yield or success in STR typing. This unexpected parity in DNA preservation between the petrous bone, a renowned source of endogenous DNA in skeletal remains, and the epiphyses of metacarpals, which are porous and susceptible to taphonomic changes, is surprising. In this study, we introduced ATR-FTIR spectroscopy as an approach to unravel the correlation between bone molecular structure and DNA preservation. Metacarpals and petrous bones with same taphonomic history were sampled and prepared for DNA analyses. While one portion of the sample was used for DNA analysis, the other underwent ATR-FTIR spectroscopic examination. The normalized spectra and FTIR indices between the epiphyses of metacarpals and petrous bones were compared. Because the taphonomic history of the remains used is relatively short and stable, the ATR-FTIR spectroscopy unveiled subtle structural differences between the two bone types. Petrous bones exhibited higher mineralization, whereas epiphyses contained more organic matter. The unexpected preservation of DNA in the epiphyses of metacarpals can likely be attributed to the presence of soft tissue remnants within the trabeculae. Here observed differences in the molecular structure of bones indicate there are different mechanisms enabling DNA preservation in skeletal tissues.
