BACKGROUND
In oculoplastic surgery, reconstruction of a large defect after the removal of a massive malignant lower lid tumor still represents a unique challenge. We will report on this case, including a presentation of the case using step ladder V-Y advancement flap.
METHODS
During November 2018 to March 2023, five patients of lower eyelid malignant tumor had wide resection with safety margin and reconstructed using step ladder V-Y advancement flap. The flap was used step ladder V-Y advancement flap.
RESULTS
No complications, including ectropion deformity, occurred. This flap does not sacrifice healthy skin as seen with the cheek rotation flap, and the area of dissection is very small and can be performed in a short time.
CONCLUSIONS
Step ladder V-Y advancement flap is highly useful in cases that require a reconstruction of a large defect after the removal of a massive malignant lower lid tumor from viewpoints of operating time, ease of procedure, aesthetics, and complications.

Nonviral transfection has been used to express various recombinant proteins, therapeutics, and virus-like particles (VLP) in mammalian and insect cells. Virus-free methods for protein expression require fewer steps for obtaining protein expression by eliminating virus amplification and measuring the infectivity of the virus. The nonviral method uses a nonlytic plasmid to transfect the gene of interest into the insect cells instead of using baculovirus, a lytic system. In this chapter, we describe one of the transfection methods, which uses polyethyleneimine (PEI) as a DNA delivery material into the insect cells to express the recombinant protein in both adherent and suspension cells.

The development of novel robotic devices specifically designed for open microsurgery leads to increasing applications in reconstructive procedures. While initial studies revealed improved precision and surgical ergonomics upon robotic assistance, surgical time was consistently observed to be increased. This study compares two robotic suturing techniques using the Symani Surgical System and RoboticScope in a preclinical setting, to further leverage the benefits of novel robotic devices in microsurgery. Six experienced microsurgeons performed three microvascular anastomoses with a "steady-thread" suturing technique and a "switch-thread" technique on 1.0-mm-diameter artificial silicone vessels. Time for anastomosis and participant's satisfaction with the techniques and robotic setup were recorded. Anastomosis quality and microsurgical skills were assessed using the Anastomosis Lapse Index and Structured Assessment of Microsurgery Skills. Lastly, technical error messages and thread ruptures were quantified. Knot tying was significantly faster and evaluated significantly better by participants using the steady-thread technique (4.11 ± 0.85 vs. 6.40 ± 1.83 min per anastomosis). Moreover, microsurgical skills were rated significantly better using this technique, while both techniques consistently led to high levels of anastomosis quality (2.61 ± 1.21 vs. 3.0 ± 1.29 errors per anastomosis). In contrast, the switch-thread technique was associated with more technical error messages in total (14 vs. 12) and twice as many unintended thread ruptures per anastomosis (1.0 ± 0.88 vs. 0.5 ± 0.69). This study provides evidence for the enhanced performance of a steady-thread suturing technique, which is suggested to be applied upon robot-assisted microsurgical procedures for optimized efficiency.

Cigarette butts (CBs) and Microplastics (MPs) have serious harmful effects on the environment and living organisms despite their small size. This research aims to investigate the abundance and pollution status of CBs and MPs in Tahtalı Dam Basin (West Anatolia, Türkiye) which is the most important drinking and irrigation water resources. Clean Environment Index (CEI) and Cigarette Butt Pollution Index (CBPI) were used to determine pollution degree of the basin. The total number of CBs were 1.478 items, the total number of MPs were 477 items/m in the basin. As a result of this study, MP particles weren't found in Balaban Stream. Highest number of MP particles observed in 100-250 μm (45%) size class. The most abundant MP type and colour were, fragment (54%) and white (42%), respectively. Polyethylene terephthalate (50%) was the most abundant type of polymer according to the ATR- FTIR analysis. As a result of the CEI and CBPI, the upstream stations of the stream were classified as "clean" status, while downstream sampling points of the stream and Balaban Lake coasts were classified as "extremely dirty" status. The calculated volumes of MP particulates from mining facility, agricultural and recreational activities indicate that anthropogenic factors are the most important MP source in the Tahtalı Dam Basin. This study is the first study about MP and CB pollution of the freshwater ecosystems in the region.

Microplastics, an emerging contaminant, are widespread in oceans around the world, and rivers are the key conveyors of these pollutants into the oceans. There exists a dearth of available data pertaining to seasonal fluctuation, spatial distribution and risk assessment of microplastics in rivers extending from upper reaches to the lower reaches. The collection of such data is of utmost importance for the purpose of formulating beneficial management strategies for riverine microplastics. In order to bridge this research gap, an investigation was made in the Periyar River in Kerala, India, which is exposed to anthropogenic stress and is at risk of microplastic pollution. A total of eighteen sites (six sites each from downstream, midstream and upstream) along the 244 km of the river were investigated across three seasons in a year. The study revealed a discernible pattern in the spatial distribution of microplastic concentrations, wherein there was a rise in abundance from the upstream to midstream and then a sudden increase of abundance along the downstream regions towards the lower reaches. The highest mean microplastic abundance of 124.95 items/L was obtained during the monsoon season followed by post-monsoon season i.e. 123.21 items/L and pre-monsoon i.e. 120.50 items/L. The predominant forms of microplastics were found to be fibres, fragments and filaments. Most prevalent polymer types acquired were polyethylene (PE) and polypropylene (PP). Pollution hazard index (PHI) and pollution load index (PLI) were also evaluated to assess the water quality of this river. The findings of this study conclude that the Periyar River is polluted with microplastics throughout its course and offer significant insights into the detection of microplastic origins in river systems and lend support to the implementation of potential measures aimed at mitigating their impact.

PURPOSE
The exit points of the supraorbital nerve and its branches hold significant importance in various medical procedures, including supraorbital endoscopic surgeries, forehead-facial aesthetic plastic surgeries, medical aesthetic applications and maxillofacial surgeries. Therefore, the primary objective of the present study was to precisely define the dimension, location and clinical significance of the supraorbital foramen/notch. By doing so, we aimed to enhance our understanding of this anatomical structure and its implications for surgical and aesthetic interventions in the supraorbital region.
METHODS
For our study, we conducted anatomical dissections and bone measurements to assess the significance of anatomical variations of the supraorbital foramen/notch. We utilized a sample consisting of 28 cadavers and 38 skulls. The supraorbital foramen/notch was bilaterally analyzed in all 28 cadavers and 38 dry bones. We morphometrically analyzed the distance between the supraorbital foramen/notch and various anatomical landmarks, including the nasion, supraorbital margin, infraorbital margin, temporal crest, glabella, frontal cavity and midline of the face. Additionally, we measured the distance between the supraorbital foramen/notches and the frontal foramen/notches, and the width of the supraorbital foramen/notch and the distance between both supraorbital foramina/notches.
RESULTS
There are 32 (57.14%) supraorbital foramina, and the remaining are 24 (42.86%) supraorbital notches in cadavers and there are 36 (47.37%) supraorbital foramina, and the remaining are 40 (52.63%) supraorbital notches in skulls. We observed consistency in the dimension and location values of anatomical measurement parameters between cadavers and dry skulls on both right and left sides, with the exception of the parameter "distance from temporal crest" (p=0.042). Furthermore, our correlation analysis revealed a significant positive relationship between the right and left sides across all parameters, except for the following instances: in dry skulls, "distance from supraorbital margin" and in cadaver parameters, "distance from temporal crest, " "distance from frontal cavity" and "width."
CONCLUSION
In our study, we observed that the distributions of supraorbital foramina and notches were nearly similar. Furthermore, our findings indicated comparable measurements between the right and left sides in both cadavers and skulls. These results suggest a degree of consistency in supraorbital anatomy within our study sample, regardless of the specimen type (cadavers or skulls) or laterality (right or left side).

BACKGROUND
Surgical chest masculinization procedures, especially gender-affirming top surgery (GATS), are becoming increasingly prevalent in the USA. While a variety of surgical techniques have been established as both safe and effective, there is limited research examining ideal aesthetic nipple appearance and incision scar pattern. This study employs patient images to understand the public's perception on top surgery outcomes when adjusting for BMI ranges and Fitzpatrick skin types.
METHODS
Images from RealSelf modified via Adobe Photoshop depicted various scar types and nipple-areolar complex (NAC) sizes/positions. A Qualtrics survey was distributed utilizing Amazon Mechanical Turk. Statistical analysis was performed through JMP Pro 17 for ordinal and categorical values, with a p value less than or equal to 0.05 statistically significant.
RESULTS
A moderately sized and laterally placed NAC was preferred. A transverse scar that resembles the pectoral border between the level of the inframammary fold and pectoral insertion was deemed most masculine and aesthetic. Majority of results demonstrated that this is unaffected by Fitzpatrick skin types. Increased BMI images impacted public preferences, as a nipple placed farther from the transverse incision (p = 0.04) and a transverse scar position closer to the IMF was preferred in higher BMI patients.
CONCLUSIONS
An understanding of the most popular NAC and scar choices, as well as how these factors may differ when considering a Fitzpatrick skin type or BMI categorization was attained. This validates the importance of patient-centered approach when employing surgical techniques in GATS. Future studies intend to obtain reports from actual patients considering GATS.

Lipids have essential functions as structural components of cellular membranes, as efficient energy storage molecules, and as precursors of signaling mediators. While deregulated glucose and amino acid metabolism in cancer have received substantial attention, the roles of lipids in the metabolic reprogramming of cancer cells are less well understood. However, since the first description of de novo fatty acid biosynthesis in cancer tissues almost 70 years ago, numerous studies have investigated the complex functions of altered lipid metabolism in cancer. Here, we will summarize the mechanisms by which oncogenic signaling pathways regulate fatty acid and cholesterol metabolism to drive rapid proliferation and protect cancer cells from environmental stress. The review also discusses the role of fatty acid metabolism in metabolic plasticity required for the adaptation to changing microenvironments during cancer progression and the connections between fatty acid and cholesterol metabolism and ferroptosis.

Changes caused by learning that a food is inedible in were examined for fast and slow synaptic connections from the buccal ganglia S1 cluster of mechanoafferents to five followers, in response to repeated stimulus trains. Learning affected only fast connections. For these, unique patterns of change were present in each follower, indicating that learning differentially affects the different branches of the mechanoafferents to their followers. In some followers, there were increases in either excitatory or inhibitory connections, and in others, there were decreases. Changes in connectivity resulted from changes in the amplitude of excitation or inhibition, or as a result of the number of connections, or of both. Some followers also exhibited changes in either within or between stimulus train plasticity as a result of learning. In one follower, changes differed from the different areas of the S1 cluster. The patterns of changes in connectivity were consistent with the behavioral changes produced by learning, in that they would produce an increase in the bias to reject or to release food, and a decrease in the likelihood to respond to food.
