Application of Photodynamic Therapy with 5-Aminolevulinic Acid to Extracorporeal Photopheresis in the Treatment of Cutaneous T-Cell Lymphoma: A First-in-Human Phase I/II Study.
Extracorporeal photopheresis (ECP) is a therapeutic modality used for T-cell-mediated disorders. This approach involves exposing isolated white blood cells to photoactivatable 8-methoxypsoralen (8-MOP) and UVA light, aiming to induce apoptosis in T-cells and thereby modulate immune responses. However, conventional 8-MOP-ECP lacks cell selectivity, killing both healthy and diseased cells, and has shown limited treatment efficacy. An alternative approach under investigation involves the use of 5-aminolevulinic acid (ALA) in conjunction with light, referred to as ALA-based photodynamic therapy. Our previous ex vivo studies suggest that ALA-ECP exhibits greater selectivity and efficiency in killing T-cells derived from patients with T-cell-mediated disorders compared to those treated with 8-MOP-ECP. We have conducted a clinical phase I-(II) study evaluating ALA-ECP safety and tolerability in cutaneous T-cell lymphoma (CTCL). Here, 20 ALA-ECP treatments were administered to one CTCL patient, revealing no significant changes in vital signs. Two adverse events were reported; both evaluated by the Internal Safety Review Committee as non-serious. In addition, five conceivable events with mainly mild symptoms took place. During the study period, a 53% reduction in skin involvement and a 50% reduction in pruritus was observed. In conclusion, the results indicate that ALA-ECP treatment is safe and well tolerated.

Lactoferrin (Lf), a multifunctional protein found abundantly in secretions, including tears, plays a crucial role in ocular health through its antimicrobial, immunoregulatory, anti-inflammatory, and antioxidant activities. Advanced delivery systems are desirable to fully leverage its therapeutic potential in treating ocular diseases. The process of Lf quantification for diagnostic purposes underscores the importance of developing reliable, cost-effective detection methods, ranging from conventional techniques to advanced nano-based sensors. Despite the ease and non-invasiveness of topical administration for ocular surface diseases, challenges such as rapid drug elimination necessitate innovations, such as Lf-loaded contact lenses and biodegradable polymeric nanocapsules, to enhance drug stability and bioavailability. Furthermore, overcoming ocular barriers for the treatment of posterior segment disease calls for nano-formulations. The scope of this review is to underline the advancements in nanotechnology-based Lf delivery methods, emphasizing the pivotal role of multidisciplinary approaches and cross-field strategies in improving ocular drug delivery and achieving better therapeutic outcomes for a wide spectrum of eye conditions.

Despite the high success rates of dental implants, peri-implantitis is currently the most common complication in dental implantology. Peri-implantitis has an inflammatory nature, it is associated with the accumulation of plaque in the peri-implant tissues, and its evolution can be progressive depending on various factors, comorbidities, and poor oral health. Prophylaxis and different treatment methods have been widely discussed in recent decades, and surgical and non-surgical techniques present both advantages and disadvantages. In this work, a literature review of different studies on the application of adjuvant treatments, such as local and systemic antibiotics and antiseptic treatments, was conducted. Positive outcomes have been found in the short (up to one year after treatment) and long term (up to ten years after treatment) with combined therapies. However, there is still a need to explore new therapies based on the use of advanced drug delivery systems for the effective treatment of peri-implantitis in the long term and without relapses. Hence, micro- and nanoparticles, implants, and injectable hydrogels, among others, should be considered in future peri-implantitis treatment with the aim of enhancing overall therapy outcomes.

Electric pylons are crucial components of power infrastructure, requiring accurate detection and identification for effective monitoring of transmission lines. This paper proposes an innovative model, the EP-YOLOv8 network, which incorporates new modules: the DSLSK-SPPF and EMS-Head. The DSLSK-SPPF module is designed to capture the surrounding features of electric pylons more effectively, enhancing the model's adaptability to the complex shapes of these structures. The EMS-Head module enhances the model's ability to capture fine details of electric pylons while maintaining a lightweight design. The EP-YOLOv8 network optimizes traditional YOLOv8n parameters, demonstrating a significant improvement in electric pylon detection accuracy with an average [email protected] value of 95.5%. The effective detection of electric pylons by the EP-YOLOv8 demonstrates its ability to overcome the inefficiencies inherent in existing optical satellite image-based models, particularly those related to the unique characteristics of electric pylons. This improvement will significantly aid in monitoring the operational status and layout of power infrastructure, providing crucial insights for infrastructure management and maintenance.

The precise segmentation of retinal vasculature is crucial for the early screening of various eye diseases, such as diabetic retinopathy and hypertensive retinopathy. Given the complex and variable overall structure of retinal vessels and their delicate, minute local features, the accurate extraction of fine vessels and edge pixels remains a technical challenge in the current research. To enhance the ability to extract thin vessels, this paper incorporates a pyramid channel attention module into a U-shaped network. This allows for more effective capture of information at different levels and increased attention to vessel-related channels, thereby improving model performance. Simultaneously, to prevent overfitting, this paper optimizes the standard convolutional block in the U-Net with the pre-activated residual discard convolution block, thus improving the model's generalization ability. The model is evaluated on three benchmark retinal datasets: DRIVE, CHASE_DB1, and STARE. Experimental results demonstrate that, compared to the baseline model, the proposed model achieves improvements in sensitivity (Sen) scores of 7.12%, 9.65%, and 5.36% on these three datasets, respectively, proving its strong ability to extract fine vessels.

Grating-type spectral imaging systems are frequently employed in scenes for high-resolution remote-sensing observations of the Earth. However, the entrance of the grating-type spectral imaging system is a slit or a pinhole. This structure relies on the push broom method, which presents a challenge in capturing spectral information of transiently changing targets. To address this issue, the IFU is used to slice the focal plane of the telescope system, thereby expanding the instantaneous field of view (IFOV) of the grating-type spectral imaging system. The aberrations introduced by the expansion of the single-slice field of view (FOV) of the IFU are corrected, and the conversion of the IFU's FOV from arcseconds to degrees is achieved. The design of a spectral imaging system based on an image-slicer IFU for remote sensing is finally completed. The system has a wavelength range of 1400 nm to 2000 nm, and a spectral resolution of better than 3 nm. Compared with the traditional grating-type spectral imaging system, its IFOV is expanded by a factor of four. And it allows for the capture of complete spectral information of transiently changing targets through a single exposure. The simulation results demonstrate that the system has good performance at each sub-slit, thereby validating the effectiveness and advantages of the proposed system for dynamic target capture in remote sensing.

This study addresses the limitations of current tonometry techniques by exploring vibroacoustic properties for estimating intraocular pressure (IOP), a key diagnostic parameter for monitoring glaucoma-a significant risk factor for vision loss. Utilizing vivo porcine eyeballs, we investigated the relationship between IOP and the nonlinear vibration transfer function ratio (NVTFR). Through applying varying vibration levels and analyzing responses with transfer function analysis and univariate regression, we identified a strong negative correlation between NVTFR and IOP, evidenced by a Pearson correlation coefficient of -0.8111 and significant results from generalized linear model (GLM) regression (-value < 0.001). These findings indicate the potential of NVTFR as a vital indicator of IOP changes. Our study highlights the feasibility of using vibroacoustic properties, specifically NVTFR, to measure IOP. While further refinement is necessary for in vivo application, this approach opens new possibilities for non-invasive and patient-friendly IOP monitoring, potentially enhancing ophthalmology diagnostic techniques and providing a foundation for future research and development in this critical area.

Conventional optical fiber temperature/strain sensors often have to make compromises between the resolution and the dynamic range. Here we present a new method that meets the measurement requirements for both high resolution and large dynamic range. A high-quality optical fiber Fabry-Perot Interferometer (FPI) constructed using a pair of chirped fiber Bragg gratings is employed as the sensor and a dual-mode direct spectrum interrogation method is proposed to identify the small drift of external temperature or strain. As a proof-of-concept illustration, a temperature resolution of 0.2 °C within 30-130 °C is demonstrated. For strain sensing, the resolution can be 10 µε within 0-1000 µε. The measurement resolution can be improved further by routinely increasing the reflectivity of the CFBG and the cavity length and the sensor can also be mass-produced. This new sensing schema not only resolves the conflict between the resolution and the dynamic range of fiber-optic temperature/strain sensors but can also be extended to other sensors and measurands.

The quality of aerial remote sensing imaging is heavily impacted by the thermal distortions in optical cameras caused by temperature fluctuations. This paper introduces a lumped parameter thermal network (LPTN) model for the optical system of aerial cameras, aiming to serve as a guideline for their thermal design. By optimizing the thermal resistances associated with convection and radiation while considering the camera's unique internal architecture, this model endeavors to improve the accuracy of temperature predictions. Additionally, the proposed LPTN framework enables the establishment of a heat leakage network, which offers a detailed examination of heat leakage paths and rates. This analysis offers valuable insights into the thermal performance of the camera, thereby guiding the refinement of heating zones and the development of effective active control strategies. Operating at a total power consumption of 26 W, the thermal system adheres to the low-power limit. Experimental data from thermal tests indicate that the temperatures within the optical system are maintained consistently between 19 °C and 22 °C throughout the flight, with temperature gradients remaining below 3 °C, satisfying the temperature requirements. The proposed LPTN model exhibits swiftness and efficacy in determining thermal characteristics, significantly facilitating the thermal design process and ensuring optimal power allocation for aerial cameras.
