Thyrolipoma or adenolipoma of the thyroid gland is defined as a thyroid adenoma containing mature fat tissue. It is a rare encapsulated lesion. A case of a 35-year-old female presenting with swelling in the neck that was diagnosed as adenolipoma is described because of its extreme rarity.

Adenolipomas are rare benign neoplasms composed of mature adipose tissue and thyroid follicles. The preoperative fine-needle aspiration cytology of such lesion has rarely been cited. Approximately 12 cases have been reported in the literature worldwide, diagnosed solely on histopathology. A 65-year-old woman presented with a 4-month history of a thyroid nodule. A FNA cytology specimen showed a few benign follicular cells with adipose tissue. Right lobectomy was performed and the specimen revealed a solid yellowish mass measuring 3 x 2.5 cm. Microscopic findings showed a solid tumor predominantly composed of mature adipose tissue intermixed with thyroid follicles. The pathological diagnosis was adenolipoma of the thyroid gland. The presence of adipose tissue is a common finding within the cytologic specimen, especially in obese individuals or with inadequate sampling. But suspicion may be possible if excess amounts of adipose tissue are present in the submitted sample.

Adenolipoma of the skin is a rare variant of lipoma not yet reported to involve the external female genitalia. We report, for the first time, 2 cases of adenolipoma arising in or immediately adjacent to the vulva in women aged 41 and 44 years. They are characterized by sparse mammaglobin immunoreactive eccrine sweat glands embedded within lobules of mature adipose tissue. This report brings attention to this novel soft tissue tumor in a site where fat and sweat glands are normal tissue constituents.

Adenolipomas are rare benign neoplasms composed of mature adipose tissue and glandular elements. We report 6 patients with adenolipomas of the head and neck region: 3 in the parathyroid, 2 in the thyroid, and 1 in the parotid gland. The patients were 4 women and 2 men, ranging in age from 21 to 70 years (mean, 62.5 years). The patients with parathyroid adenolipomas manifested signs and symptoms of hyperparathyroidism, whereas the others complained of a mass in the neck. Computed tomography and magnetic resonance imaging are useful tools for the preoperative diagnosis of adenolipomas of the head and neck. The findings of adipose tissue and benign glandular elements by fine-needle aspiration biopsy should prompt consideration of adenolipoma in the differential diagnosis. Complete surgical excision is curative.

Adenolipoma is a rare microscopic variant of cutaneous lipoma composed of large lobules of mature adipocytic tissue admixed with eccrine ducts and glands. We report a new case of cutaneous adenolipoma, and the first showing an apocrine cystic glandular component. This lesion may not be a specific entity, and could represent entrapment of epithelial structures within a lipoma.

Adenolipoma of the skin is an unusual variant of lipoma recently described by Hitchcock et al. and characterized by the presence of normal eccrine sweat glands within a lipoma. We report a case and review the literature. A 45-year-old woman presented with a slow-growing, painless nodule on the thigh, clinically considered to be lipoma. Microscopically it comprised an adipose-tissue proliferation with a single eccrine secretory coil and associated duct in the periphery and in the center of the nodule. This benign lesion has been termed adenolipoma because of the presence of adipose tissue and eccrine glands. It probably represents only a histological curiosity in which the eccrine glands are entrapped by the adipose proliferation. Adenolipoma of the skin is a distinct lesion that can occur in the dermis or subcutaneous tissue.

We report a case of adenolipoma of the thyroid gland, which is very unusual because thyroid adenoma rarely contains mature adipose tissue. A 55-year-old Japanese woman presented with a 1-month history of a neck mass. A blinded fine-needle aspiration biopsy showed benign follicular cells with no adipose cells, and we performed a left hemithyroidectomy. The specimen measured 7.0 x 4.4 cm and weighed 59.9 g. The cut surface of the tumor revealed a solid yellowish mass, and hematoxylin-eosin staining showed that the solid tumor was well separated from the adjacent thyroid tissue by a thin fibrous capsule. The tumor was found to be follicular adenoma composed of epithelial cells arranged in monotonous microfollicular patterns, intermingled with mature adipose tissue. No adipose tissue was found outside the tumor, and there were no signs of amyloid deposits in the tumor or adjacent thyroid tissue. The pathological diagnosis was adenolipoma of the thyroid gland. We discuss the pathogenesis of this entity and review the previously documented cases.
