The antihypertensive action of Prazosin was studied in 20 hypertensive patients. Sixteen patients completed the 36-week treatment period. The drug produced a gradual and sustained fall in both systolic and diastolic blood pressure. The dosage used was 1 mg TDS. Only 2 patients required 2 mg TDS of Prazosin combined with 1 mg per day of polythiazide, and one of these did not respond. No postural hypotension or weight gain was observed. Side-effects were mild and were not troublesome.

Clinical Trial Comparative Study
The study was conducted in 118 patients with cardiac and pulmonary lesions, aged 18 to 75 years. Among the diuretics the leading role belongs to Uregyt, and Lasix, as judged by their natriuretic and diuretic action, then follow Nuvorit, Hypothiaside and Brinaldix. Renese (polythiaside) produces the same diuretic effect as Nuvorit and Hypothiaside. Renese, Hypothiaside, Lasix and Uregyt enhance kaliuresis and often reduce potassium blood plasma concentrations, while Nuvorit and Brinaldix increase kaliuresis least of all, and often increase the potassium blood plasma level. Within two days of discontinuing Thriamterene nearly all body potassium that was retained under the effect of this drug is excreted. Nuvorit, Hypothiaside, Renese, Brinaldix, Lasix, Uregyt cause similar changes in the acid-base balance (respiratory and metabolic alkalosis), and produce no effect upon the acid-base balance rather seldom. Lasix and Uregyt cause metabolic alkalosis more often than other diuretics. Apart from that, Hypothiaside also causes respiratory acidosis in 1/4 of the patients.

An open trial of prazosin has been conducted among 18 males aged 18-68 years (mean 42.7) and 22 females aged 23-70 years (mean 47.5) seen at the Lagos University Teaching Hospital Hypertension Clinic. 3 suffered from mild to moderate hypertension and 9 suffered from severe hypertension. The dose employed ranged from 2 to 60 mg per day in 2 or 3 divided doses. All 6 patients whose placebo diastolic pressure was below 100 mm Hg gave excellent response. 19(76%) of the 25 patients whose placebo diastolic pressure ranged from 100 to 114 mm Hg gave excellent response. Only 3 to 9 patients whose placebo diastolic pressure ranged from 115 to 129 mm Hg gave excellent response. There was improved response when polythiazide 2 mg daily was added to the treatment. The total number giving excellent response increased from 28 (70%) to 36 (90%). In 3 cases (7.5%), the response was fair and one case (2.5%) remained poor. In most cases there was slight increase in pulse rate. The side effects encountered included palpitation in 8 patients 3 of whom also had postural hypotension. There were no biochemical or haematological abnormalities caused by prazosin but on continued therapy 16 patients developed tolerance to its effect. Thus prazusin has been found to be useful addition to the antihypertensive arm amentarium.

The administration of a single dose of furosemide, ethacrynic acid and polythiazide to healthy individuals under conditions of maximum water diuresis produces a significant increase in renal magnesium excretion. Elevated Mg excretion displayed a direct correlation to renal sodium excretion after furosemide (r=0.689, p less than 0.001), ethacrynic acid (r=0.869, p less than 0.001) and polythiazide (r=0.586, p less than 0.01). The slopes of the various regression lines did not differe significantly from each other or from the slope of the regression line characterizing this correlation for mannitol (r= 0.603, p less than 0.01). A significant linear correlation was likewise found between the excretion of Mg and total osmotically active substances after furosemide (r=0.783, p less than 0.001), ethacrynic acid (r=0.88, p less than 0.001) and polythiazide (r=0.646, p less than 0.01). The regression lines of the given correlations did not differ significantlyfrom each other, but their slopes were significantly higher than that of the regression line for the correlation after mannitol (r=0.454, p less than 0.01). The findings indicate that tubular Mg transport is influenced both by a decrease in tubular Na resorption in the diluting segment (polythiazide) and by an effect on Na resorption in the parts of the nephron proximal to the diluting segment of the nephron (furosemide, ethacrynic acid).

The efficacy of prazosin given either alone or in combination with polythiazide was assessed in a study of 50 outpatients with essential hypertension. Among the 29 who completed 42 months of therapy, blood pressure fell significantly in 76%. Of the 21 who dropped out of the study after 1 to 41 1/2 months, 43% showed improvement, 38% showed no change, and 19% showed worsening of hypertension. The most common side effects were lack of energy and weakness. Diabetics and alcoholics tolerated prazosin well. Of the 50 patients who started treatment with prazosin alone, only 23 required addition of polythiazide.
