Despite an increasing surge of exosome use throughout the aesthetic arena, a paucity of published exosome-based literature exists. Exosomes are membrane-bound extracellular vesicles derived from various cell types, exerting effects via intercellular communication and regulation of several signaling pathways. The purpose of this review was to summarize published articles elucidating mechanisms and potential applications, report available products and clinical techniques, and prompt further investigation of this emerging treatment within the plastic surgery community.
METHODS
A literature review was performed using PubMed with keywords exosomes, secretomes, extracellular vesicles, plastic surgery, skin rejuvenation, scar revision, hair growth, body contouring, and breast augmentation. Publications from 2010 to 2021 were analyzed for relevance and level of evidence. A Google search identified exosome distributors, where manufacturing/procurement details, price, efficacy, and clinical indications for use were obtained by direct contact and summarized in table format.
RESULTS
Exosomes are currently derived from bone marrow, placental, adipose, and umbilical cord tissue. Laboratory-based exosome studies demonstrate enhanced outcomes in skin rejuvenation, scar revision, hair restoration, and fat graft survival on the macro and micro levels. Clinical studies are limited to anecdotal results. Prices vary considerably from $60 to nearly $5000 based on company, source tissue, and exosome concentration. No exosome-based products are currently Food and Drug Administration-approved.
CONCLUSIONS
Administered alone or as an adjunct, current reports show promise in several areas of aesthetic plastic surgery. However, ongoing investigation is warranted to further delineate concentration, application, safety profile, and overall outcome efficacy.

Introduction The use and consumption of "products", such as cosmetic procedures and confectionery, is on a rise in the Danish population. However, it has never been evaluated if the same tendency can be observed among the providers of these products. In view of the upcoming Christmas, we decided to investigate this delicate matter. Methods This study was a survey-based cross-sectional study examining demographics, professional backgrounds, as well as frequency and preferences regarding confectionery and cosmetic procedures among confectioners and plastic surgeons. Results A total of 90 persons answered the questionnaire. Results showed that consumption of confectionery was high among both confectioners and plastic surgeons, and that the use of cosmetic procedures was higher among both confectioners and plastic surgeons than among the background population. Both groups preferred to know the person who provided "the product" within their respective area. Conclusion Most plastic surgeons do not need to know their confectioner but would like to know their cosmetic procedure provider. So, if you, as part of the department management, or simply as a well-meaning colleague, want to treat your staff/colleagues, you can safely order cake from a random confectioner but not cosmetic procedures from a random surgeon! Hand injuries are common, and due to the complex functions of the hand, soft tissue defects present a surgical challenge in reconstruction. Hand defects exposing deeper structures warrant reconstruction with local flaps, but in Denmark, reconstruction surgery of the hand is routinely managed within the orthopaedic specialty alone with no plastic surgical involvement. This review aims at describing the most common local flaps of the hand and forearm and encourage to stronger collaboration between hand- and plastic surgeons in Denmark.

Chat Generative Pre-Trained Transformer (ChatGPT) is a versatile large language model-based generative artificial intelligence. It is proficient in a variety of tasks from drafting emails to coding to composing music to passing medical licensing exams. While the potential role of ChatGPT in plastic surgery is promising, evidence-based research is needed to guide its implementation in practice. This review aims to summarize the literature surrounding ChatGPT's use in plastic surgery. A literature search revealed several applications for ChatGPT in the field of plastic surgery, including the ability to create academic literature and to aid the production of research. However, the ethical implications of using such chatbots in scientific writing requires careful consideration. ChatGPT can also generate high-quality patient discharge summaries and operation notes within seconds, freeing up busy junior doctors to complete other tasks. However, currently clinical information must still be manually inputted, and clinicians must consider data privacy implications. Its use in aiding patient communication and education and training is also widely documented in the literature. However, questions have been raised over the accuracy of answers generated given that current versions of ChatGPT cannot access the most up-to-date sources. While one must be aware of its shortcomings, ChatGPT is a useful tool for plastic surgeons to improve productivity for a range of tasks from manuscript preparation to healthcare communication generation to drafting teaching sessions to studying and learning. As access improves and technology becomes more refined, surely more uses for ChatGPT in plastic surgery will become apparent.

Supermicrosurgery was popularised in 1997 and is defined as a technique of microvascular anastomosis for single nerve fascicles and vessels 0.3-0.8 mm in diameter. It requires the use of powerful microscopes, ultradelicate microsurgical instruments and specialist dyes. The development of supermicrosurgery has vastly improved the ability of microsurgeons to create true perforator flaps with minimal donor site morbidity for reconstructive surgery and improved the precision of additional microsurgical techniques. This review outlines the origins and history of supermicrosurgery, its current applications in reconstructive surgery (including fingertip reconstructions, true perforator flap surgery, nerve flaps and lymphoedema surgery), supermicrosurgery training and future directions for the field.

This abstract presents an overview of the applications, challenges, and potential benefits of machine learning in plastic surgery. One of the key areas where machine learning has been applied is in the preoperative assessment and surgical planning process. By analyzing large datasets of patient images and clinical data, machine learning algorithms can assist plastic surgeons in predicting surgical outcomes, identifying optimal surgical techniques, and minimizing potential complications. These algorithms can learn from past cases and provide valuable insights to improve surgical decision-making and optimize patient care. Furthermore, machine learning has shown promise in facial recognition and analysis, which is crucial in plastic surgery procedures involving the face. Algorithms can accurately detect facial landmarks, assess facial symmetry, and simulate potential surgical outcomes. This technology gives plastic surgeons a more comprehensive understanding of the patient's facial structure and aids in designing personalized treatment plans. Additionally, machine learning algorithms have been employed to automate the analysis of large-scale clinical databases, assisting in identifying patterns, risk factors, and treatment outcomes. By leveraging these algorithms, plastic surgeons can gain valuable insights into patient populations, surgical trends, and postoperative complications. This information can inform clinical decision-making, improve patient safety, and enhance the overall quality of care. Despite the numerous advantages, several challenges need to be addressed when integrating machine learning into plastic surgery. These include the need for high-quality and diverse datasets, algorithm interpretability, ethical considerations, and regulatory compliance.
