Review
The present study was aimed at reviewing the studies that used finite element analysis (FEA) to estimate the biomechanical stress arising in removable partial dentures (RPDs) and how to optimize it. A literature survey was conducted for the English full-text articles, which used only FEA to estimate the stress developed in RPDs from Jan 2000 to May 2021. In RPDs, the retaining and supporting structures are subjected to dynamic loads during insertion and removal of the prosthesis as well as during function. The majority of stresses in free-end saddle (FES) RPDs are concentrated in the shoulder of the clasp, the horizontal curvature of the gingival approaching clasp, and the part of the major connector next to terminal abutments. Clasps fabricated from flexible materials were beneficial to eliminate the stress in the abutment, while rigid materials were preferred for major connectors to eliminate the displacement of the prosthesis. In implant-assisted RPD, the implant receive the majority of the load, thereby reducing the stress on the abutment and reducing the displacement of the prosthesis. The amount of stress in the implant decreases with zero or minimal angulation, using long and wide implants, and when the implants are placed in the first molar area.

Dentures are an excellent treatment modality for partial and edentate patients; however, improper denture care and hygiene can result in both decreased longevity of the prosthesis and increased risk of developing dental caries, periodontal disease and oral candidosis. Previously, it has been shown that patients and dental professionals are unaware of the different materials and methods available for optimum denture care and hygiene. This article provides an overview of the key legislation and main commercially available methods for denture cleanliness and hygiene, and serves as a basis for providing tailored denture hygiene for denture wearers.

A relatively common oral health problem, gag reflex is a clinical challenge during dentures fabrication. In the current literature several techniques have been described regarding this clinical condition. The latter use either psychological, pharmacological and prosthetic therapies to treat these patients. The purpose of this case report is to highlight an original prosthetic strategy which consists on a progressive lengthening of the maxillary complete prosthesis allowing a patient with acute gag reflex to adapt to the denture palatal extension.

Whilst increasing numbers of older people retain natural teeth, a sizable proportion of older people are still edentate. Replacement of missing teeth is important for function, aesthetics and to facilitate social interactions. The process of providing oral rehabilitation for edentate older adults can be challenging, but clinicians can utilise a number of treatment options including removable or implant retained prostheses. In this article, complete denture construction will be described using both conventional and copy denture techniques. Recent innovations in the process for constructing complete dentures using computer aided design and manufacturing (CAD-CAM) techniques will also be discussed. Dental implants can be used to help retain removable complete prostheses for edentate patients and the use of implant-retained overdentures is presented as a treatment alternative.

Review
The wear of dentures carries with it the possibility of an inflammatory response by the denture-bearing tissues. Lack of or improper hygiene has been shown to contribute to denture stomatitis (DS). Although essential, denture hygiene is often overlooked by patients or performed improperly. As professional caregivers dentists must instruct patients and ensure that they understand the relevance and importance of denture hygiene. In light of this, the authors conducted a critical review of the literature on protocols and procedures to highlight the importance of denture hygiene and clinically show the effects of lack of denture care. A literature search was done through Google, PubMed, and Google Scholar that focused on publications published in English that dealt with denture hygiene and the disinfection process and on the protocols used. Relevant articles for protocols for denture disinfections were reviewed, and the results of different disinfection techniques were assessed, including manual, chemical, microwave, and a combination of techniques. The authors conclude that dentists must take time to instruct patients on proper denture disinfection procedures and the importance of cleaning the dentures well at least once daily. Denture hygiene and removal prior to sleeping is beneficial in preventing DS and allowing the tissue to achieve a state of homeostasis.
