Endoscopic atraumic coronary artery bypass grafting (EndoACAB) is a novel approach in the treatment of ischemic heart disease compared to traditional coronary artery bypass grafting (CABG). Analyzing the early and late outcome of EndoACAB is very important. EndoACAB was performed in "Tbilisi Heart and Vascular Clinic" for the first time in Georgia in 2015. The study aim was to analyze early postoperative period results and compare them with the research carried out at some leading international cardiothoracic centers. 162 patients underwent EndoACAB in 2015-2017. We studied preoperative and intraoperative characteristics of patients as well as early postoperative outcome. No intraoperative mortality was detected. Lethal outcome in early postoperative period was seen in 1,2%. Intraoperative switch from Endo-ACAB to CABG was seen in 2,5%. Pneumon was diagnosed in 0.6%, atrial fibrillation and flutter - 1,9%, no surgical wound infection nor stroke was detected. mean days of hospital stay was 4,8 days±1.2. There was no statistically significant difference in our results and mortality rate compared to the results of foreign referral centers. We assume that EndoACAB which is a novelty for Georgian medical society is successfully implemented and its early outcome is the same as operations performed in leading international centers.

The authors outline a review of preoperative, intraoperative, and postoperative considerations surrounding adult spinal deformity. Preoperative management topics include imaging, hemoglobin A1c levels before spine surgery, osteoporotic management, and prehabilitation. Topics surrounding intraoperative management include the use of antibiotics, liposomal bupivacaine, and Foley catheters. The authors also discuss postoperative questions surrounding analgesia, nausea and vomiting, thromboembolic prophylaxis, and early mobilization. Throughout their discussion, the authors incorporate enhanced recovery after surgery protocols to hopefully lead to future discussions regarding optimizing complex spinal patients.

PURPOSE
General anesthesia may affect the quality of postoperative sleep, especially after surgery on elderly patients. The decline of postoperative sleep quality may produce harmful effects on the postoperative recovery of patients. In this review, we summarized the efficacy and potential mechanism of acupuncture on postoperative sleep quality.
METHODS
We review the effect of general anesthesia on circadian sleep rhythm. In addition, to provide evidence about the impairment of decreased postoperative sleep quality, we also emphasize the mechanism of acupuncture alleviates factors that affect sleep quality after general anesthesia.
RESULTS
The application of acupuncture technology has been helpful to improve sleep quality and alleviate postoperative complications affecting postoperative sleep quality after general anesthesia.
CONCLUSION
Acupuncture at different acupoints could effectively improve body's neurotransmitter levels and regulate biological clock genes through various mechanisms, and then improve postoperative sleep quality. Large-scale multi-center trials are needed to verify these findings.

INTRODUCTION
Oxygen saturation and lactate are markers of tissue hypoxia; they are obtained from central venous and mixed venous sample of the pulmonary artery. The simultaneous behavior of these parameters in the postoperative period of cardiac surgery is unknown.
OBJECTIVE
To characterize the lactate and oxygen saturation of the venous-arterial circuit of the postoperative patient from cardiac surgery.
METHODS
Design: Analytical cross-sectional. In consecutive patients after cardiac surgery, serum lactate and oxygen saturation of the venous-arterial circuit were obtained. The variables were reported with median (25.75 percentiles). They were analyzed with Kruskal-Wallis ANOVA and respective adjustment, Spearman correlation, the descriptive Bland-Altman statistic and intraclass correlation coefficient (95% confidence interval). A p < 0.05 was considered significant.
RESULTS
244 blood samples from 61 patients were studied. Women 30 (49%). (Oxygen saturation) [lactate] were: arterial 98 (95.3, 99.4%) and 1.7 (1.1, 2.1); peripheral venous 85 (75.4, 94%) and [1.9 (1.35, 2.3)]; central venous 68.8 (58.74, 70.2%) and 1.8 (1.3, 2.3); mixed central venous 66.8 (61.2, 73.1%) and 1.8 (1.3, 2.2), p < 0.05. The best intraclass correlation coefficient for oxygen saturation were from central vein to mixed central vein 0.856 (0.760,0.914); and lactate: 0.954 (0.923, 0.972).
CONCLUSIONS
The oxygen saturation differs in the venous-arterial circuit unlike lactate where they are similar. The best values of the intraclass correlation coefficient for lactate and oxygen saturation were those obtained in central vein and mixed central vein.
