Response times (RTs) are commonly used to assess cognitive abilities, though it is unclear whether face processing RTs predict recognition ability beyond accuracy. In the current study, we examined accuracy and RT on a widely used face matching assessment modified to collect meaningful RT data, the computerized Benton Facial Recognition Test (BFRT-c), and measured whether RTs predicted face recognition ability and developmental prosopagnosia (DP) vs. control group membership. 62 controls and 36 DPs performed the BFRT-c as well as validated measures of face recognition ability: the Cambridge Face Memory Test (CFMT) and a Famous Faces Memory Test (FFMT). In controls, BFRT-c accuracy robustly predicted CFMT (r = .49, < .001), FFMT (r = .43, < .001), and a CFMT-FFMT composite (r = .54, < .001), whereas BFRT-c RT was not significantly associated with these measures (all r's .21). We also found that BFRT-c accuracy significantly differed between DPs and controls, but RT failed to differentiate the groups.

Individuals with developmental prosopagnosia (DP) all exhibit impairments in face memory, but the specificity of these face memory impairments is debated. One problem is that standard behavioural tasks are not able to provide independent measurement of face perception, face memory, and face matching (the decision process required to judge whether two instances of a face are of the same individual or different individuals). The present study utilised a new test of face matching, the Oxford Face Matching Test (OFMT), and a novel analysis strategy to derive these independent indices. Twenty-nine individuals with DP and the same number of matched neurotypical controls completed the OFMT, the Glasgow Face Matching Test, and the Cambridge Face Memory Test. Results revealed individuals with DP exhibit impairments in face perception, face memory and face matching. Collectively, these results suggest that face processing impairments in DP are more comprehensive than has previously been suggested.

Developmental prosopagnosia (DP) is a condition that indicates the inability to recognize individuals by their faces from birth, without any history of brain damage. The assessment of face recognition ability and diagnosis of DP involve the use of face tests such as the Cambridge Face Memory Test (CFMT) and the Cambridge Face Perception Test, along with self-reported measures like the 20-Item Prosopagnosia Index (PI20). Face recognition accuracy is affected by anxiety. However, previous studies on the relationship between face recognition ability and anxiety have not used the PI20 measure. This study aimed to investigate the relationship between self-reported measures of face recognition ability and anxiety tendencies among healthy young individuals for DP diagnosis and its implications. We used a face recognition test, involving the PI20, CFMT, Visual Perception Test for Agnosia-Famous Face Test (VPTA-FFT), and State-Trait Anxiety Inventory (STAI). We assessed the performance of 116 Japanese young adults (75 females, median age of 20.7 years, with a standard deviation of 1.2). Subsequently, we conducted a statistical analysis to examine the relationship between the outcomes of the face recognition tests and STAI scores using Pearson correlation analysis and single correlation coefficients. The results showed a positive correlation between state anxiety and PI20 (r = 0.308, p = 0.007), and a weak positive correlation was also observed between trait anxiety and PI20 (r = 0.268, p = 0.04). In contrast, there was no correlation between CFMT and VPTA-FFT with respect to STAI. The results of the hierarchical multiple regression analysis also suggested that the correlation between the performance on the PI20 (self-report) and objective measures of face recognition performance (the CFMT and the VPTA-FFT) are driven by differences in anxiety. This study is the first to explore the relationship between face recognition abilities and anxiety using the PI20 self-report measure. There are implications for future research on the diagnosis of DP and the relationship between anxiety and face recognition.

Converging lines of research suggests that many developmental prosopagnosics (DPs) have impairments beyond face perception, but currently no framework exists to characterize these impaired mechanisms. One potential extra-perceptual deficit is that DPs encode/retrieve faces in a distinct manner from controls that does not sufficiently support individuation. To test this possibility, 30 DPs and 30 matched controls performed an old/new face recognition task while providing confidence ratings, to which a model-based ROC analysis was applied. DPs had significantly reduced recollection compared to controls, driven by fewer 'high-confidence target' responses, but intact familiarity. Recollection and face perception ability uniquely predicted objective and subjective prosopagnosia symptoms, together explaining 51% and 56% of the variance, respectively. These results suggest that a specific deficit in face recollection in DP may represent a core aspect of the difficulty in confidently identifying an individual by their face.

The organisational principles of the visual ventral stream are still highly debated, particularly the relative association/dissociation between word and face recognition and the degree of lateralisation of the underlying processes. Reports of dissociations between word and face recognition stem from single case-studies of category selective impairments, and neuroimaging investigations of healthy participants. Despite the historical reliance on single case-studies, more recent group studies have highlighted a greater commonality between word and face recognition. Studying individual patients with rare selective deficits misses (a) important variability between patients, (b) systematic associations between task performance, and (c) patients with mild, severe and/or non-selective impairments; meaning that the full spectrum of deficits is unknown. The Back of the Brain project assessed the range and specificity of visual perceptual impairment in 64 patients with posterior cerebral artery stroke recruited based on lesion localization and not behavioural performance. Word, object, and face processing were measured with comparable tests across different levels of processing to investigate associations and dissociations across domains. We present two complementary analyses of the extensive behavioural battery: (1) a data-driven analysis of the whole patient group, and (2) a single-subject case-series analysis testing for deficits and dissociations in each individual patient. In both analyses, the general organisational principle was of associations between words, objects, and faces even following unilateral lesions. The majority of patients either showed deficits across all domains or in no domain, suggesting a spectrum of visuo-perceptual deficits post stroke. Dissociations were observed, but they were the exception and not the rule: Category-selective impairments were found in only a minority of patients, all of whom showed disproportionate deficits for words. Interestingly, such selective word impairments were found following both left and right hemisphere lesions. This large-scale investigation of posterior cerebral artery stroke patients highlights the bilateral representation of visual perceptual function.

Apperceptive Prosopagnosia Secondary to an Ischemic Infarct of the Lingual Gyrus: A Case Report and an Update on the Neuroanatomy, Neurophysiology, and Phenomenology of Prosopagnosia.
Phenomenology is the philosophical study of experience and its core feature of sentience, the very ability to be conscious of a sensation and how we perceive it. Nowhere is this idea more vivid, as in the phenomenon of vision and the ability to form and sense a visual percept. The clinical entity of prosopagnosia, the ability to sense but not recognize a face, strikes at the heart of this phenomenon. We describe a classic case of selective apperceptive prosopagnosia due to an ischemic infarct of the left occipital lobe with extension to the lingual gyrus. It is well-established that acquired prosopagnosia usually involves the right more than the left occipital cortex, with localization of lesions bilaterally more than unilaterally. The ischemic infarcts strategically involve the fusiform gyrus, inferior occipital gyrus, the fundus of the posterior temporal sulcus, parahippocampal gyrus, and, rarely, lingual gyrus, which is almost always not a solitary finding. We seize upon this opportunity to explore the concept of visual prosopagnosia and outline the latest ideas on the neuroanatomical localization, neurophysiology, and classification of this intriguing phenomenon.

Prosopagnosia describes the inability to recognize others by their faces, which may be hereditary or acquired. Acquired cases result from intracranial lesions such as intracranial hemorrhage or ischemia. This case demonstrates acquired prosopagnosia secondary to an intracranial hemorrhage and thus exemplifies the importance of early symptom recognition for appropriate diagnosis and management. A 58-year-old female presented to the emergency department with a chief complaint of the worst headache of her life along with nausea and vomiting. She also reported that she was unable to recognize her children in photos and although she knew her husband was with her, she did not recognize his face. Physical examination revealed no focal motor deficits. Computed tomography angiography of the brain revealed intracerebral hemorrhage of the right occipital lobe. Acquired prosopagnosia can be the only presenting symptom of intracranial pathology. It is most commonly caused by intracranial hemorrhage, as shown in this case report. This demonstrates a unique symptom of posterior circulation strokes that are commonly misdiagnosed in the emergency department.

Face Recognition Ability (FRA) varies widely throughout the population. Previous research highlights a positive relationship between self-perceived and objectively measured FRA in the healthy population, suggesting that people do have insight into their FRA. Given that this relationship has not been investigated in Italian samples yet, the main aim of the present work was to develop an Italian translation of the Prosopagnosia Index-20 (PI-20), a self-report measure of FRA, to investigate the relationship between PI-20 performances and an objective assessment given by the Cambridge Face Memory Test Long Form (CFMT+) in the Italian population. A sample of 553 participants filled in the PI-20 Italian version 1 or 2 (PI-20_GE or PI-20_BA) and completed the CFMT+. Results showed a negative correlation between both versions of the Italian PI-20 and CFMT+ scores, meaning that the more self-evaluations were negative, the worse they objectively performed. The same results applied to the extreme limits of the distribution (i.e., 10% of the highest and lowest PI-20 scores). Furthermore, both age and administration order of the tests were predictor variables of CFMT+ scores. Overall, our results suggest that people possess insight, although relatively limited, into their FRA.

In non-clinical populations, facial features (eyes, nose, mouth) may vary in their contribution to face identity perception. Changes to whole faces are easier to detect than changes to individual features, and eye changes are typically easier to detect than mouth changes, which in turn are easier to detect than nose changes. However, how this differs for people with face recognition difficulties (developmental prosopagnosia; DP) and for individuals with superior face recognition abilities (super-recognisers; SR) is not clear; although findings from previous studies have suggested differences, the nature of this difference is not understood. The aim of this study was to examine whether differences in the ability to detect feature changes in DPs and SRs were (a) quantitative, meaning that the pattern across feature changes remained the same but there was an overall upwards or downwards shift in performance, or (b) qualitative, meaning that the pattern across feature changes was different. Using a change detection task in which individual face features (eyes, nose, mouth) changed between sequentially presented faces, we found that while prosopagnosics showed a quantitative difference in performance with a downwards shift across all conditions, super-recognisers only showed qualitative differences: they were better able to detect when the face was the same and were marginally (but not non-significantly) worse at detecting when the eyes changed. Further, the only condition which distinguished between the three groups was the ability to identify when the same face was presented, with SRs being better than controls, and controls being better than DPs. Our findings suggest that, in feature-matching tasks, differences for DPs are due to them being overall worse at the task, while SRs use a qualitatively different strategy.

Many studies have attempted to identify the perceptual underpinnings of developmental prosopagnosia (DP). The majority have focused on whether holistic and configural processing mechanisms are impaired in DP. However, previous work suggests that there is substantial heterogeneity in holistic and configural processing within the DP population; further, there is disagreement as to whether any deficits are face-specific or reflect a broader perceptual deficit. This study used a data-driven approach to examine whether there are systematic patterns of variability in DP that reflect different underpinning perceptual deficits. A group of individuals with DP (N = 37) completed a cognitive battery measuring holistic/configural and featural processing in faces and non-face objects. A two-stage cluster analysis on data from the Cambridge Face Perception Test identified two subgroups of DPs. Across several tasks, the first subgroup (N = 21) showed typical patterns of holistic/configural processing (measured via inversion effects); the second (N = 16) was characterised by reduced or abolished inversion effects compared to age-matched control participants (N = 91). The subgroups did not differ on tasks measuring upright face matching, object matching, non-face holistic processing, or composite effects. These findings indicate two separable pathways to face recognition impairment, one characterised by impaired configural processing and the other potentially by impaired featural processing. Comparisons to control participants provide some preliminary evidence that the deficit in featural processing may extend to some non-face stimuli. Our results demonstrate the utility of examining both the variability between and consistency across individuals with DP as a means of illuminating our understanding of face recognition in typical and atypical populations.
